Pre-eclampsia following chemotherapy for breast cancer during pregnancy: case report and review of the literature

Abstract: Whenever possible, pregnant patients with breast cancer should receive the same treatment as those who are not pregnant. Should chemotherapy for breast cancer be indicated in pregnancy from the second trimester onwards only, contraindications would be other risks for pre-eclampsia and intrauterine growth restriction, such as smoking and gestational diabetes.

“…Only one major malformation (i.e., pyloric stenosis) was described with the use of docetaxel administered in the second and third trimester [57]. A total of four pregnancies in patients exposed to docetaxel experienced reduction in amniotic fluid: however, trastuzumab was co-administered in two of them [58,59] and the third pregnancy was complicated by placental insufficiency [60]. A high rate of small for gestational age birth weights has been observed in children exposed to docetaxel [28].…”

“…19 Oxidative stress, which is one of the proposed pathophysiological mechanism of preeclampsia, can also be induced by cytotoxic agents. 20 However, the frequency of pre-eclampsia did not increase when chemotherapy was given during pregnancy. The seemingly high rate of caesarean sections might be explained by the number of preterm deliveries and the recommendation to deliver before the 35th week of gestation, as per guidelines in some countries (eg, Germany).…”

“…Regarding the pre-eclampsia, development of this disease was expected in front of many risk factors as its age, the medically assisted reproduction, and the triple gestation [59]. The lack of the pre-eclampsia prevalence increase in case of chemotherapy is highlighted, int an section for a HELLP syndrome at 36 WA but in this patient, pre-eclampsia appears after the 2 cycles of chemotherapy [60]. In addition, Del Gobbo et repercussions in these 23 cases [62].…”
