Praecancerose und Carcinoma in situ

From 249 histologically examined cases with ulcerative colitis (within the years 1967 to 1974) 20 cases were selected, which had an onset of the disease before the 20th year of age and/or a duration of more than 10 years. These cases were checked-up for "prae-cancerous" epithelial dysplasia. Four of these 20 cases showed medium to severe epithelial dysplasia together with carcinoma (= 1.61% of the entire collective). In 2 cases low grade to severe dysplasia occurred in different parts of the colon without demonstrable carcinoma. In 1 case epidermoid epithelial metaplasia were found in the splenic flexure and in the rectum. The biological significance of these metaplasia remains unclear. With reference to the formal pathogenesis these lesions are probably "indirect" metaplasia. The early diagnosis of "colitis"-carcinoma can be improved by systematic colonoscopic examinations resp. by colonoscopic biopsies. "Prae-cancerous" epithelial dysplasias, which occur in a high rate together with carcinoma, are found not only in the rectum but also in other parts of the colon. Multiple biopsies from different parts of the colon as well as the rectum would thus seem to be desirable if mucosal sampling is to be employed as a screening test.

Nowadays the resected stomach is considered to be in a precancerous state in the broader sense. Until now there have been no systematic, histoautoradiographic investigations concerning the proliferative kinetics of the resected stomach in the region of anastomosed mucosa sections. Histoautoradiographic investigations should give an answer to the following questions: 1. In which way do the proliferative kinetics of gastric mucosa in the part of the anastomosis change with increasing postoperative interval? 2. Is there a result of variations in the proliferative kinetics of the anastomotic distant mucosa of the fundus? To find an answer to these questions, 75 Wistar-rats out of 231 operated rats (Billroth's 1st principle) were selected for histoautographic analysis. The laboratory animals were killed at fixed points of time in a postoperative interval of 104 weeks of experiment and the gastrointestinal preparations were worked up and interpreted for the histoautographic analysis. During the postoperative period the 3H-labelling-indices showed a characteristic course in the part of the gastric anastomosis. After a continuous increase of the 3H-labelling indices up to the fourfold of standard values two years after the operation there was a slight decrease of the proliferative activity. But it still remained twice the normal value. In addition, extended displacements and decompositions of gastric anastomosis mucosa could be observed. However, the proliferative activity in the anastomotid distant mucosa remained unchanged. A cell proliferation of gastric anastomosis increasing considerably and demonstrated in our experiment has to be considered as promotion factor of cancer development. Anastomotic carcinomas developing during the total experiment give evidence for this.

The adequate appraisal of precancerous changes is of the utmost importance in our everlasting fight against cancer, and its socio–rnedical relevance is acknowledged world–wide. In every country where epidemiologists register a rising incidence of malignant neoplasms, endeavours must focus not only on the early diagnosis of existing cancers, but even more on detecting the signs of coming tumors. That is why today the precancerous lesions are a main point of interest in cancer research all over the world for pathologists, radiologists, surgeons, obstetricians, etc. ACTA PATHOL.
