Practical management of antibiotic allergy in adults

Abstract: This review looks at the main issues around immediate hypersensitivity and the role and limitations of testing. The majority of literature on antibiotic hypersensitivity relates to β-lactam antibiotics, mainly because of the heavy usage of this class of drugs. Concerns around cross-reactivity always worry clinicians, particularly in the emergency situation. Reasonable data now exist in relation to β-lactam antibiotics and derivatives, which enable appropriate risk management to be undertaken. The available lit… Show more

“…3. Algorithms of use of cephalosporins in patients with reported penicillin allergy [81] If it is impossible to replace the antibiotic, which probably is the cause of the allergic reaction, a challenge is required [30,80,82]. Contraindications to challenge are Stevens-Johnson or Lyell's syndrome at an earlier age.…”

“…equal to the therapeutic dose, for the next administration in the event of a negative result. If the patient experienced severe anaphylactic reactions within the latest year, a challenge starts with 1/1,000 of the singular therapeutic dose [30,82]. Despite a rather detailed description of the challenge technique by foreign authors, unfortunately, it is not possible to use it routinely in Russian clinical practice, as there are no legally binding Russian documents (i.e.…”

“…Incorrect penicillin allergy records are common, as side effects and symptoms of illness can be mistaken for allergic reaction symptoms (1). About 6% of the UK general practice population has a record of a penicillin allergy but fewer than 10% of these patients are likely to be truly allergic (2).…”
