Introduction: The recent circulation of arboviruses transmitted by vectors, such as dengue, chikungunya, and Zika, is concerning due to the high morbidity rates, clinical complications, and increased demand on health services. The objective of this study was to analyze the clinical and epidemiological aspects of an epidemic caused by arboviruses in the municipality of Santa Luzia, Minas Gerais, Brazil. Methods: Longitudinal study of patients with acute febrile disease and suspected arbovirus infection reported to Brazilian Notifiable Disease Information System (Sistema de Informação de Agravos de Notificação) from the epidemiological week 44 of 2015 to epidemiological week 52 of 2016. Patients with confirmed chikungunya were followed-up for 18 months and those with Zika for 15 months. Additionally, we analyzed and described the temporal distribution of confirmed cases of these arboviruses in this municipality. Results: Overall 3,531 arboviruses cases, including 3,481 (98.7%) cases of dengue, 38 (1.0%) cases of chikungunya, and 12 (0.3%) cases of Zika were confirmed. The highest incidence of arbovirus infection occurred in the first quarter of 2016 (epidemiological week 7 to 14). The most frequent symptoms were for dengue, which included fever, headache, retro-orbital pain, and exanthema. Chikungunya infection was associated with fever, myalgia, arthralgia, and rash while Zika infection with pruritus and rash. Conclusions: Given the similarities in the initial clinical profiles of these arboviruses, it is important to perform a detailed clinical analysis, laboratory diagnosis, and patient follow-up.

Introdução: A Febre do Chikungunya é uma enfermidade febril aguda emergente que acarreta alta morbidade. Objetivo: Este estudo objetiva descrever os aspectos clínicos e epidemiológicos da introdução do Vírus Chikungunya (CHIKV)em Sergipe. Métodos: Trata-se de um estudo ecológico, com dados secundários do Sistema Nacional de Agravos de Notificação (SINAN) e do Laboratório Central de Sergipe (LACEN/SE), entre 2015-2017. Estudou-se variáveis sociodemográficas e clínicas dos casos confirmados de Chikungunya residentes no estado de Sergipe. Os dados foram tabulados pelo TABWIN e exportados para o Excel® 2016, para análise estatística utilizou-se o OpenEpi e teste qui-quadrado (p<0,05). Resultados: Demonstrou-se rápida expansão da enfermidade no território sergipano, atingindo nos 3 anos 82,7% dos municípios, com maior acometimento das microrregiões de saúde Grande Aracaju e Agreste Central. Dos 8806 casos confirmados verificou-se predominância em mulheres (60,79%), indivíduos de 20 a 59 anos (63,04%), pardos (27,46%), residentes em zona urbana (51,95%). Febre (83,8%), mialgia (77,9%) e artralgia (63,1%) estão entre os sintomas com maior frequência. Conclusão: O CHIKV apresenta grande potencial de disseminação no território e sua sintomatologia pode ser confundida com outros quadros infecciosos. Faz-se necessário o aprofundamentodo conhecimento sobre os diversos aspectos envolvidos no controle, diagnóstico e manejo clínico.

Background: Infectious disease epidemiology and planetary health literature often cite solid waste and plastic pollution as risk factors for vector-borne diseases and urban zoonoses; however, no rigorous reviews of the risks to human health have been published since 1994. This paper aims to identify research gaps and outline potential solutions to interrupt the vicious cycle of solid wastes; disease vectors and reservoirs; infection and disease; and poverty. Methods: We searched peer-reviewed publications from PubMed, Google Scholar, and Stanford Searchworks, and references from relevant articles using the search terms ("disease" OR "epidemiology") AND ("plastic pollution," "garbage," and "trash," "rubbish," "refuse," OR "solid waste"). Abstracts and reports from meetings were included only when they related directly to previously published work. Only articles published in English, Spanish, or Portuguese through 2018 were included, with a focus on post-1994, after the last comprehensive review was published. Cancer, diabetes, and food chain-specific articles were outside the scope and excluded. After completing the literature review, we further limited the literature to "urban zoonotic and biological vector-borne diseases" or to "zoonotic and biological vector-borne diseases of the urban environment." Results: Urban biological vector-borne diseases, especially Aedes-borne diseases, are associated with solid waste accumulation but vector preferences vary over season and region. Urban zoonosis, especially rodent and canine disease reservoirs, are associated with solid waste in urban settings, especially when garbage accumulates over time, creating burrowing sites and food for reservoirs. Although evidence suggests the link between plastic pollution/solid waste and human disease, measurements are not standardized, confounders are not rigorously controlled, and the quality of evidence varies. Here we propose a framework for solutions-based research in three areas: innovation, education, and policy. Krystosik et al. Solid Wastes and Disease Vectors and Reservoirs Conclusions: Disease epidemics are increasing in scope and scale with urban populations growing, climate change providing newly suitable vector climates, and immunologically naïve populations becoming newly exposed. Sustainable solid waste management is crucial to prevention, specifically in urban environments that favor urban vectors such as Aedes species. We propose that next steps should include more robust epidemiological measurements and propose a framework for solutions-based research.
