Potency Preserving Cystectomy With Intrafascial Prostatectomy for High Risk Superficial Bladder Cancer

Abstract: The main criticism about so-called sexuality sparing cystectomy has been the presence of consistent prostatic remnants. Performing intrafascial prostatectomy together with supra-ampullar cystectomy seems to warrant good functional results with while better preserving oncological safety.

“…In our limited series, we did not consider the influence of the nerve-sparing procedure on postoperative QOL since we handled the patients who had undergone orthotopic neobladder creation in the early period of the neobladder era. Recently, Puppo et al [20] reported a new technique for potency-preserving radical cystectomy with intrafascial prostatectomy. Median IIEF-5 decreased extremely slightly postoperatively with respect to the baseline (25 vs. 21), while maintaining good oncological outcomes.…”

“…This procedure was completed similarly to that firstly described by Puppo et al [10] in 2008 and subsequently referred in different series by Ong et al [11] and Hautmann et al [12]. The main steps include a blunt extraperitoneal antegrade dissection between the posterior bladder wall and the seminal vesicles up to the ejaculatory ducts and a retrograde intrafascial prostatectomy.…”

“…A recent review of 252 cancer patients who had some form of prostate-sparing cystectomy also found a suspiciously high rate of distant metastases [160]. The risk of ED following surgery varied in different series depending on the extent of surgery [157][158][159][161][162][163].…”
