Posttraumatic stress and anxiety in patients with an implantable cardioverter defibrillator: Trajectories and vulnerability factors

Abstract: Comparable trajectories were found for symptoms of PTSD and anxiety over time. Patients with a younger age, increased depression score at baseline, and Type D personality were especially prone to experience PTSD and anxiety symptomatology at 12 months follow-up. Due to the negative association between PTSD, anxiety, and health outcomes, it is paramount to timely identify the subset of patients who are vulnerable to experience these symptoms and provide them with personalized support.

“…ICD activity can invoke fear in patients to the extent they develop conditions akin to a posttraumatic stress disorder . The spectrum of any such adverse effects is unique in each individual, and while they may relate to preexisting personality traits of implantees or their later acquired concerns, these responses are unpredictable . Other midterm risks include the occurrence of inappropriate shocks which affect as many as a quarter of recipients, sometimes related to technical factors such as oversensing, and which may of itself confer a mortality risk .…”

“…The acceptance of the ICD appears to be influenced by a number of variables such as psychological morbidity, advanced age, severity of the disease, onset of anxious or depressive symptoms, type D personality, worry about the ICD implantation, and understanding of the benefits and disadvantages of the ICD, which would seem to impact on the perceived quality of life [241][242][243][244].…”
