Summary Postoperative sore throat has a reported incidence of up to 62% following general anaesthesia. In adults undergoing tracheal intubation, female sex, younger age, pre‐existing lung disease, prolonged duration of anaesthesia and the presence of a blood‐stained tracheal tube on extubation are associated with the greatest risk. Tracheal intubation without neuromuscular blockade, use of double‐lumen tubes, as well as high tracheal tube cuff pressures may also increase the risk of postoperative sore throat. The expertise of the anaesthetist performing tracheal intubation appears to have no influence on the incidence in adults, although it may in children. In adults, the i‐gel™ supraglottic airway device results in a lower incidence of postoperative sore throat. Cuffed supraglottic airway devices should be inflated sufficiently to obtain an adequate seal and intracuff pressure should be monitored. Children with respiratory tract disease are at increased risk. The use of supraglottic airway devices, oral, rather than nasal, tracheal intubation and cuffed, rather than uncuffed, tracheal tubes have benefit in reducing the incidence of postoperative sore throat in children. Limiting both tracheal tube and supraglottic airway device cuff pressure may also reduce the incidence.

Background. Postoperative sore throat (POST) is a common complication after general anaesthesia. It affects patient satisfaction and can affect activity after discharge. The supraglottic airway device (SAD) offers an alternative to traditional tracheal intubation with potential benefit in preventing sore throat. Numerous agents have been used as lubricant to reduce the incidence of POST with variable efficacy. The purpose of this study is to determine the prevalence of postoperative sore throat in patients with Virgin Coconut Oil (VCO) as a lubricant for Laryngeal Mask Airway (LMA) insertion in patients undergoing general anaesthesia for elective surgery. Objective. Determine the prevalence of postoperative sore throat in patients by giving Virgin Coconut Oil (VCO) as a lubricant for the Laryngeal Mask Airway (LMA) installation under general anesthesia for elective surgery. Methods. This is an observational descriptive study, was conducted in December 2019 until June 2020 at Surgical Installation RSUP dr. M. Djamil Padang and Andalas University Hospital. Forty-two subjects were recruited to this study taken by using consecutive sampling technique. Results. Patients evaluated about sore throat at immediately, 2 hours, and 24 hours post-operative period. POST was observed in 47,6% of the patients in immediate post-operative period. After 2 hours 23,8% patients had sore-throat, and after 24 hours 9,5% patients had sore-throat. Among patients had sore throat after 24 hours, the intensity was mild. Conclusion. We conclude that lubricating cuff of LMA with VCO can reduce the prevalence of POST. Keyword: Laryngeal Mask Airway, sore throat, Virgin Coconut Oil

Background Post-operative sore throat, cough, and hoarseness of voice constitute the major pharyngolaryngeal morbidities following General anesthesia with an endotracheal tube since its introduction. Pharyngolaryngeal morbidity has been reported following the use of supraglottic airway devices as well, with less frequency and severity. Lidocaine jelly, a time-tested lubricating agent with local anesthetic effects is effective in reducing the incidence and severity of pharyngolaryngeal morbidity. Steroid gel application over the endotracheal tube is an effective alternative. The aim of this work is to compare betamethasone gel and lidocaine jelly in their effects leading to the causation of pharyngolaryngeal morbidity when applied to I-gel, a commonly used supraglottic airway device in practice now. Results Both betamethasone gel and Lidocaine jelly were found to be equally efficacious in controlling pharyngolaryngeal morbidity following I-gel insertion. Though the incidence of post-operative sore throat (POST) was lower in the B group in the first 2 h (P=0.895) and 6 h (P=0.582) postoperatively, it was not significant. Similar results with cough (P=0.362) and hoarseness of voice (P=0.123) found after 2 h were also not statistically significant. Conclusions Both betamethasone gel and lidocaine jelly reduced the incidence and severity of pharyngolaryngeal morbidity following I-gel insertion and was found comparable. Trial registration CTRI/2017/10/010058. Registered 11th October 2017.
