Postirradiation sarcoma (malignant fibrous histiocytoma) of axilla

Abstract: A case is reported of a patient who developed a histologically unusual sarcoma in the axilla and chest wall 8 years after receiving radiation therapy (6500 rad) for carcinoma of the breast. This sarcoma showed light-and electronmicroscopic features of a malignant fibrous histiocytoma, a tumor not documented among 24 previously reported cases of postirradiation sarcoma following the diagnosis of breast carcinoma. In addition, the literature is reviewed and discussed regarding postirradiation sarcoma in general … Show more

“…MFHs are rare tumors, often characterized by an aberration in 7cen-q22 (38% of postirradiation MFHs). [57][58][59][60][61][62] Our incidence of MFH (1 of 27 patients with RIS) was lower than in other studies. 55,[63][64][65][66][67] For instance, Laskin et al 68 detected 36 MFHs among 53 patients with RIS.…”

“…27,29,30,41,46 -53 The use of megavoltage has been associated with an increased incidence of RT-induced bone sarcomas, 54 but we only encountered three bone sarcomas. Hardy et al 57 predominantly found bone sarcomas and fibrosarcomas of the chest wall. Souba et al 58 found 7 bone sarcomas and 6 MFHs among 16 patients with RIS.…”

“…From these observations they postulated that the tumor originates from undifferentiated mesenchymal cells which show a dual differentiation toward fibroblasts and histiocytes (macrophages). The presence of intermediate cells at the ultrastructural level has been considered evidence in support of this theory (1,2,16,18,50,52). Again, however, the idea that mesenchymal cells differentiate toward both fibroblasts and histiocytes does not concur with the current idea of macrophage ontogeny.…”

“…The introduction of the more modern concept that macrophages are a descendant of hematopoietic stem cells led some researchers to question the histiocytic origin of MFH. By electron microscopic studies these researchers reported the presence of undifferentiated cells and intermediate cells that contained both rough endoplasmic reticulum and phagolysosomes (1,2,16,18,50,52). From these observations they postulated that the tumor originates from undifferentiated mesenchymal cells which show a dual differentiation toward fibroblasts and histiocytes (macrophages).…”

“…Size greater than 5 cm, high histologic grade, and presence of metastases at time of diagnosis were associated with a worse prognosis [1]. Most MFH occurred in the deep soft tissue of the extremities and trunk; only two cases of MFH of the axilla have been reported [2,3].…”
