Posttraumatic stress disorder (PTSD) develops in a subset of individuals upon exposure to traumatic stress. In addition to well-defined psychological and behavioral symptoms, some individuals with PTSD also exhibit elevated concentrations of inflammatory markers, including C-reactive protein, interleukin-6, and tumor necrosis factor-α. Moreover, PTSD is often co-morbid with immune-related conditions, such as cardiometabolic and autoimmune disorders. Numerous factors, including lifetime trauma burden, biological sex, genetic background, metabolic conditions, and gut microbiota, may contribute to inflammation in PTSD. Importantly, inflammation can influence neural circuits and neurotransmitter signaling in regions of the brain relevant to fear, anxiety, and emotion regulation. Given the link between PTSD and the immune system, current studies are underway to evaluate the efficacy of anti-inflammatory treatments in those with PTSD. Understanding the complex interactions between PTSD and the immune system is essential for future discovery of diagnostic and therapeutic tools.

: Along with other canonical systems, the renin-angiotensin system (RAS) has shown essential roles in stress. This system is a complex regulatory proteolytic cascade composed of various enzymes, peptides, and receptors. Besides the classical (ACE/Ang II/AT 1 receptor) and the counter-regulatory (ACE2/Ang-(1-7)/Mas receptor) RAS axes, evidence indicates that non-classical components, including Ang III, Ang IV, AT 2, and AT 4, can also be involved in stress. Objective and methods: This comprehensive review summarizes the current knowledge on the participation of RAS components in different adverse environmental stimuli stressors, including air-jet stress, cage switch stress, restraint stress, chronic unpredictable stress, neonatal isolation stress, and post-traumatic stress disorder. Results and conclusion: In general, activating the classical RAS axis potentiates stress-related cardiovascular, endocrine, and behavioral responses, while the stimulation of the counter-regulatory axis attenuates these effects. Pharmacological modulation in both axes is optimistic, offering promising perspectives for stress-related disorders treatment. In this regard, angiotensin-converting enzyme inhibitors and angiotensin receptor blockers are potential candidates already available because they block the classical axis, activate the counter-regulatory axis, and are safe and efficient drugs.

A PTSD ezen betegségek melletti megjelenésének számos következménye detektálható, kiemelkedő jelentőségű mind a szívelégtelen mind a transzplantált populációban észlelt non-adherenciával való összefüggése, amelynek potenciális következménye a mortalitás növekedése (25,26). A traumatikus élmény a szemtanúknál is okozhat PTSD-t, így a klinikai gyakorlat során érdemes gondolni a hozzátartozók értintettségének lehetőségére is (ICD-sokk, reanimáció).…”

A szívelégtelen beteg életútját számos ponton befolyásolja pszichés állapota és szociális helyzete. Ezen tényezők és a fizikai betegség egymásra hatásai hozzájárulnak a beteg együttműködési készségéhez (adherencia), ezen keresztül is hatva a betegség kimenetelére, a beteg életminőségére. Ugyanakkor, ezen faktorok interdiszciplináris voltuknál fogva a mindennapi gyakorlatban nehezen értékelhetők és kezelhetők az orvos szempontjából, ebben segítséget nyújthat a társszakmákkal való szoros együttműködésen alapuló ellátási forma. Jelen összefoglaló célja az adherenciát befolyásoló lelki és szociális faktorok áttekintése a szívelégtelenségtől a szívtranszplantációig, multidiszciplináris szemléletben.
