Background and Purpose Range-of-motion (ROM) exercises may contribute to hemiparetic shoulder pain, but the underlying mechanisms are unknown. This study examined scapular and humeral movement patterns in people with hemiparesis post stroke as they performed commonly prescribed ROM exercises. Methods Using kinematic techniques, we studied 13 people with hemiparesis, both with and without pain, as they performed three commonly prescribed ROM exercises: person-assisted ROM, self-assisted ROM, and cane-assisted ROM. Their data were compared to a group of 12 matched control subjects performing scapular plane shoulder elevation using mixed model ANOVAs. Correlation analyses were used to examine the relationship between participants’ ratings of pain and kinematic data. Results The hemiparetic group had mild pain at rest that increased during the performance of the exercises. During shoulder elevation, humeral external rotation in the hemiparetic group was decreased in all three ROM exercises compared to the control group. Scapular upward rotation in the hemiparetic group was decreased for the person-assisted ROM exercise only. No differences in scapular tilt were found between groups. The extent of movement abnormalities was not related to pain severity. Discussion and Conclusions People with hemiparesis had altered scapular and humeral movement patterns and increased shoulder pain when performing the ROM exercises. These data can assist clinicians in making decisions regarding which exercises to prescribe to preserve shoulder motion and prevent contractures in this population.

Our results suggest that, in subacute and chronic patients with DOC, PT could have an impact on patients' spasticity and muscles contractures. Beside PT, other factors such as time since onset and medication seem to influence the development of muscle contractures. These findings support the need for frequent PT sessions and regular re-evaluation of the overall spastic treatment for patients with DOC.

AbstrakPenurunan fungsi ekstremitas atas merupakan komplikasi yang sering terjadi pada pasien pasca stroke yang mengalami hemiplegia sebagai akibat dari kelemahan dan keterbatasan rentang gerak sendi pada bahu. Akupresur bermanfaat dalam memperbaiki fungsi ektremitas atas dengan melancarkan pergerakan aliran qi (energi vital) di dalam tubuh namun belum banyak penelitian yang mengkaji pengaruh akupresur untuk meningkatkan kekuatan otot dan rentang gerak ekstremitas atas pada pasien pasca stroke. Penelitian ini bertujuan untuk mengidentifikasi pengaruh akupresur terhadap kekuatan otot dan tentang gerak ekstremitas atas pada pasien stroke pasca rawat inap. Penelitian ini menggunakan quasi-experimental design dengan pendekatan pre-post test design pada 34 responden (n kontrol= n intervensi= 17). Kelompok intervensi diberi akupresur setiap hari 10 menit selama tujuh hari. Hasil penelitian menunjukkan adanya perbedaan yang bermakna pada kekuatan otot dan rentang gerak ekstremitas atas antara kelompok intervensi dan kelompok kontrol (p= 0,001 dan p= 0,000; α= 0,05). Akupresur merupakan intervensi yang efektif untuk meningkatkan kekuatan otot dan rentang gerak pada pasien pasca stroke yang mengalami hemiparesis. Rekomendasi pada penelitian ini adalah diperlukan adanya perawat yang menguasai akupresur dan memodifikasi standar asuhan keperawatan dengan memasukkan terapi komplementer akupresur dalam asuhan keperawatan pasien stroke yang mengalami kelemahan dan keterbatasan rentang gerak ekstremitas atas.Kata kunci: akupresur, ekstremitas atas, hemiparesis, kekuatan otot, rentang gerak, stroke (n control = n intervention = 17).
