Portal Hypertensive Duodenopathy Manifesting as “Kissing” Duodenal Ulcers in a Nigerian with Alcoholic Cirrhosis: A Case Report and Brief Review of the Literature

Abstract: Multiple duodenal ulcers are an uncommon finding in portal hypertensive duodenopathy (PHD). They represent a potential source of clinically significant bleeding from the upper gastrointestinal system in patients with cirrhosis. As this particular ulcer entity in relation to PHD has no distinguishing symptoms aside from those relating to the consequent bleeding, most of them are found either on routine endoscopic screening for cirrhotics or on endoscopic examination for cause(s) of bleeding in this patient popu… Show more

“…These uncommon extragastric lesions occur particularly in patients with extrahepatic portal hypertension [71] . Portal hypertensive duodenopathy has been defined as the endoscopic appearance of patchy or diffuse congestion of duodenal mucosa associated with portal hypertension [40] , but a consensus definition is not established [273] . Histologically, vascular changes predominate, including capillary angiogenesis, dilatation and congestion, as well as fibrous proliferation and apoptosis [46] .…”

“…The association of PHD in patients with prior variceal band ligation is very significant, and this relationship is well documented [ 2 , 11 ]. One such study showed an increase in the number of patients with PHD from 6.6% to 46.7% after variceal band ligation (p < 0.001) [ 12 ].…”

“…Treatment of PHD is not established, and recommendations are based on strategies documented in case reports [ 5 , 11 , 13 ]. Medical therapy which includes prophylaxis with beta blocker has been notably effective [ 13 ].…”
