Background Patients with residual poliomyelitis can have advanced degenerative arthritis of the hip in the paralytic limb or the nonparalytic contralateral limb. Although THA is a treatment option for some of these patients, there are few studies regarding THA in this patient population. Questions/purposes We therefore reviewed a group of patients with residual poliomyelitis who underwent cementless THA on either their paralytic limb or nonparalytic limb to assess (1) Harris hip scores, (2) radiographic results, including implant loosening, (3) complications, including dislocation, and (4) limb length discrepancy after recovery from surgery. We prospectively followed 191 consecutive collarless polished tapered (CPT) femoral stems, implanted in 175 patients who had a mean age at operation of 64.5 years (21 to 85). At a mean follow-up of 15.9 years (14 to 17.5), 86 patients (95 hips) were still alive. The fate of all original stems is known. The 16-year survivorship with re-operation for any reason was 80.7% (95% confidence interval 72 to 89.4). There was no loss to follow-up, with clinical data available on all 95 hips and radiological assessment performed on 90 hips (95%). At latest follow-up, the mean Harris hip score was 78 (28 to 100) and the mean Oxford hip score was 36 (15 to 48). Stems subsided within the cement mantle, with a mean subsidence of 2.1 mm (0.4 to 19.2). Among the original cohort, only one stem (0.5%) has been revised due to aseptic loosening. In total seven stems were revised for any cause, of which four revisions were required for infection following revision of the acetabular component. A total of 21 patients (11%) required some sort of revision procedure; all except three of these resulted from failure of the acetabular component. Cemented acetabular components had a significantly lower revision burden (three hips, 2.7%) than Harris Galante uncemented components (17 hips, 21.8%) (p < 0.001).The CPT stem continues to provide excellent radiological and clinical outcomes at 15 years following implantation. Its results are consistent with other polished tapered stem designs.

We compared the differences in bone mineral density (BMD) adjacent to two biomechanically different cementless femoral stems (Spotorno and Evolution-K). Measurements were performed within the first week after surgery and 3, 6, 12, and 24 months postoperatively in a prospective study of 31 patients with 37 total hip arthroplasties. A modified Harris hip score and a visual analogue score for patient satisfaction was used to evaluate the clinical outcome. For both stems, dual Xray absorptiometry measurements revealed a decrease in BMD in Gruen zone 7. Patients with a Spotorno stem also had a significant decrease in BMD in zones 1 and 2. In patients with an Evolution-K stem, there was a positive correlation between the clinical outcome and BMD in Gruen zones 1 and 4. Our data suggest that periprosthetic bone loss is equal to if not less pronounced adjacent to the Evolution-K stem as compared to the Spotorno stem.RØsumØ Nous avons comparØ les diffØrences dans les densitØs minØrales osseuses (DMO) au niveau de deux tiges fØmorales sans ciment de conception biomØcanique diffØrente (Spotorno et Evolution-K). Pour les deux tiges l'examen d'absorptiomtre biphotonique a rØvØlØ une baisse de DMO dans les zones 7 de Gruen. Les malades avec une tige Spotorno avaient aussi une baisse importante de DMO dans les zones 1 et 2. Chez les malades avec une tige Evolution-K il y avait une corrØlation positive entre le rØsultat clinique et la DMO dans les zones 1 et 4 de Gruen. Nos donnØes suggrent que la perte minØrales osseuses est Øquivalente au voisinage des deux tiges prothØtiques.
