Population-Based Analysis of Incidence and Outcome of Transformed Non-Hodgkin's Lymphoma

Abstract: The annual risk of transformation of FL is 3% continuing without plateau beyond 15 years. Advanced stage at diagnosis is predictive of future transformation. Clinically diagnosed transformation has an equal impact on outcome as biopsy proven transformation.

“…The rate of transformation of adult FL is low and was estimated to be in the range of 3% per year. 7 Transformation in adults is usually associated with a very poor outcome. 6,7 Here again, pediatric FL differs from the adult counterpart, in that the presence of a DLBCL component does not indicate an adverse outcome.…”

“…1,5 In adults transformation of a low-grade lymphoma to a high-grade lymphoma, usually a DLBCL, is accompanied by rapid clinical progression and an unfavorable prognosis. 6,7 However, clinicopathological studies addressing the question of a simultaneous DLBCL component and outcome of FL in children have not been published so far. We have recently shown that pediatric DLBCL differ from adult DLBCL with regard to prognosis, immunophenotype and genetics.…”

“…2 Most patients experience an indolent course, but there is significant heterogeneity in outcomes in FL, and approximately 15% of patients succumb to FL within the first 2 years after diagnosis. 2 A number of clinical prognostic scores have been developed, such as The Follicular Lymphoma International Prognostic Index (FLIPI), which accounts for several clinical features of the disease, and accurately stratifies patients into prognostic risk groups over a 5-to 10-year period. 3 However, the use of the FLIPI score does not currently dictate treatment decisions for individual patients.…”

“…A recent population based analysis from the British Columbia Cancer Agency (BCCA) including 600 newly diagnosed FL patients showed an annual risk of transformation (RT) of 3% continuing beyond 15 years of follow up. 1 The treatment of FL patients is markedly heterogeneous, as no optimal treatment approach has yet been established. The number of prior treatments, tumor burden, advanced-stage and high-risk FLIPI or IPI are the only established clinical features associated with RT.…”
