The aim of the study: to determine the structure of dorsalgia and comorbid diseases and the possibilities of rehabilitation in elderly patients with degenerative-dystrophic diseases of the spine (DDSD). The results of observation of 120 elderly patients who received sanatorium treatment in the Urgench sanatorium during 2020-21, aged 60-74 years, were analyzed, including 71 women, 49 men. All patients underwent a neurological examination, X-ray, CT or MRI of the spine, to identify comorbid diseases - ECG, ultrasound of internal organs, consultations with a general practitioner, urologist or gynecologist. According to the literature data and the results obtained, when selecting elderly patients with dorsalgia for treatment, it is necessary to take into account the “red flags” and refer such patients to specialized specialists. In addition, other comorbid diseases are identified in the elderly, which may limit the treatment of drug and physiotherapy (rehabilitation) therapy, which must be considered. Finally, in the elderly, it is necessary to take into account the so-called “yellow flags” - cognitive and depressive manifestations that affect the subjective sensation of pain.

Проблема поиска радикальной терапии множественной миеломы (ММ) до настоящего времени остается нерешенной. Заболевание характеризуется прогрессирующим течением и необходимостью про-ведения повторных курсов терапии с использованием препаратов, не обладающих перекрестной резистент-ностью. Прогноз заболевания с «двойной рефрактер-ностью» к ключевым противоопухолевым препара-там -ингибиторам протеасом первого поколения и иммуномодулирующим средствам -остается пессими-стичным. Медиана выживаемости без прогрессирова-ния (ВБП) и общей выживаемости (ОВ) в этой когорте больных составляет 5 и 9 мес. соответственно. Цель. Оценить эффективность и переносимость по-малидомида в комбинации с низкими дозами дексаме-тазона при рецидивах и рефрактерных формах мно-жественной миеломы (РРММ), осложненной «двойной рефрактерностью». Материалы и методы. С сентября 2015 г. по июль 2016 г. на базе ФГБУ «Гематологический научный центр» МЗ РФ и ФГБУ «Северо-Западный федеральный медицин-ский исследовательский центр им. В.А. Алмазова» МЗ РФ в исследование по протоколу включено 10 больных РРММ. Медиана возраста составила 62,5 года (диапазон 48-76 лет), а предшествующих линий терапии -4 (диа-пазон 3-5). У всех больных имело место прогрессиро-вание заболевания после применения бортезомиба, леналидомида, режимов с включением алкилирующих препаратов. Кроме того, 6 (60 %) из 10 больным прове-дена высокодозная химиотерапия мелфаланом с после-дующей аутоТГСК. Медиана курсов терапии -6 (диапа-зон 4-15). Результаты. Общий ответ составил 60 %, минималь-ный ответ/стабилизация заболевания -40 % (критерии IMWG). Медиана ВБП составила 7,8 мес.; ОВ за 18 мес. -
