Проблема поиска радикальной терапии множественной миеломы (ММ) до настоящего времени остается нерешенной. Заболевание характеризуется прогрессирующим течением и необходимостью про-ведения повторных курсов терапии с использованием препаратов, не обладающих перекрестной резистент-ностью. Прогноз заболевания с «двойной рефрактер-ностью» к ключевым противоопухолевым препара-там -ингибиторам протеасом первого поколения и иммуномодулирующим средствам -остается пессими-стичным. Медиана выживаемости без прогрессирова-ния (ВБП) и общей выживаемости (ОВ) в этой когорте больных составляет 5 и 9 мес. соответственно. Цель. Оценить эффективность и переносимость по-малидомида в комбинации с низкими дозами дексаме-тазона при рецидивах и рефрактерных формах мно-жественной миеломы (РРММ), осложненной «двойной рефрактерностью». Материалы и методы. С сентября 2015 г. по июль 2016 г. на базе ФГБУ «Гематологический научный центр» МЗ РФ и ФГБУ «Северо-Западный федеральный медицин-ский исследовательский центр им. В.А. Алмазова» МЗ РФ в исследование по протоколу включено 10 больных РРММ. Медиана возраста составила 62,5 года (диапазон 48-76 лет), а предшествующих линий терапии -4 (диа-пазон 3-5). У всех больных имело место прогрессиро-вание заболевания после применения бортезомиба, леналидомида, режимов с включением алкилирующих препаратов. Кроме того, 6 (60 %) из 10 больным прове-дена высокодозная химиотерапия мелфаланом с после-дующей аутоТГСК. Медиана курсов терапии -6 (диапа-зон 4-15). Результаты. Общий ответ составил 60 %, минималь-ный ответ/стабилизация заболевания -40 % (критерии IMWG). Медиана ВБП составила 7,8 мес.; ОВ за 18 мес. -
Based on some previous results related to the dynamics of various offshore structures, the present paper is focused on the evaluation of the dynamics of a typical offshore system, known as WindFloat Concept, taking into consideration the specific conditions on a location in the Black Sea. The capability to be used for wind energy recuperation is investigated using the seakeeping performances in terms of response amplitude operators (RAO’s) of motions, exciting forces and accelerations. To this purpose, two different 3D computer codes have been used. The intention wasn’t to evaluate comparatively the merits of the two applications, but to check the eventual differences which could appear and which are the parameters which could significantly influence the results. As a preliminary investigation, different theoretical approaches are available taking into account structure’s shape, water depth, appropriate wave theory on the location, etc. As a matter of fact, the complexity of such evaluations leads to very deep investigations related to the influence of different factors to be taken into account during the design stages, starting with preliminary ones when the main parameters have to be considered. From this point of view, some influences, like damping coefficient values, number of panels used for discretization, etc., are evaluated and important observations are made in order to be able to take the necessary precautions for practical applications. When floating structures are considered, a preliminary analysis of behaviour of the station keeping system (mooring, dynamic positioning, etc.) is mandatory to be carried out considering the “stiffness” of the system. Mention should be made that in the present paper only free floating condition is investigated. The results are finally compared with the source of excitation forces, using the Black Sea Spectrum for a range of wind speeds. The importance of these preliminary evaluations is to have a first consistent evaluation of the utilization of the concept, adapted to the specific of the location and to provide reliable input data for further analysis.
Treatment of female SAMP-1 mice with Neuronol (drug containing succinic acid) given with drinking water starting from the age of 2 months during the whole life prolonged the lifespan and markedly reduced mortality of animals aged 1.5-2 years. Neuronol inhibited the development of spontaneous tumors, primarily lymphomas, and significantly prolonged lifespan in mice with tumors. Long-term treatment with Neuronol had no pathological side effects. Our experiments demonstrated geroprotective and anticarcinogenic activity of Neuronol and safety of its long-term use.
31Несмотря на значительные достижения в области мо-лекулярной онкологии, биохимии и патофизиологии опу-холевых клеток и исследований механизмов взаимодей-ствия опухоли с окружающими тканями, наблюдаемые в последнее десятилетие, существенного прорыва в соз-дании высокоселективных, таргетных молекул, способ-ных полностью вытеснить из обращения системные ци-тостатические лекарственные препараты, не произошло. Удельный вес таргетных препаратов в структуре противо-опухолевой терапии остается невысоким, что объясняется не только их высокой стоимостью, но и слабой корреля-цией между впечатляющими результатами, достигнутыми на экспериментальном этапе изучения, и наблюдениями в клинической практике [1][2][3]. Таким образом, исполь-зование в лечении новообразований классических си-стемных противоопухолевых средств и проблема их углу-бленного клинического изучения остаются актуальными в настоящее время.Основными направлениями развития химиотерапии являются увеличение избирательности ее действия на злокачественные клетки и ассоциированные с ними эле-менты стромы опухоли и минимизация системного воз-действия используемых препаратов на различные органы и ткани, напрямую не вовлеченные в патологический процесс. При этом различные осложнения лекарственной терапии опухолей остаются весьма распространенным яв-лением в клинической практике, накладывают серьезные ограничения на тактику лечения пациентов и ухудшают прогноз заболевания.Наиболее остро эти вопросы стоят в отношении при-менения доксорубицина -представителя группы ан-трациклиновых антибиотиков, высокая эффективность которого в отношении широкого спектра опухолей под-тверждена в ходе многочисленных экспериментальных и клинических исследований [4,5] Цель исследования -оценка ангиопротективного действия натрия-L-аргинина сукцината (унифузол, ооо «нтФФ По-ЛиСАн», россия) при острой интоксикации доксорубицином в эксперименте. Материал и методы. Эффективность пре-парата оценивали по его влиянию на баланс вазодилатирующих и вазоконстрикторных соединений, соотношению про-и антивоспалительных цитокинов и степени выраженности апоптоза эндотелиоцитов. Результаты. Показано, что натрия-L-аргинина сукцинат улучшает эндотелиозависимую вазодилатацию и уменьшает выраженность системных проявлений воспаления, индуцированных введением доксорубицина. Заключение. Выраженное эндотелиопротективное действие ис-следуемого препарата является патогенетическим обоснованием для дальнейшего изучения унифузола в качестве ком-плексного средства для кардио-и ангиопротекции при патологии сердечно-сосудистой системы различной этиологии.
