Oral lichen planus (OLP) is among the most common oral diseases. Its etiopathogenesis has yet to be clearly identified. OLP patients complain of mouth dryness. This study aimed to assess the level of Mucin 5B in OLP patients with xerostomia. This study was conducted on 30 OLP patients and 30 healthy individuals. In addition to patient complaint of mouth dryness, xerostomia was assessed by tongue blade and lipstick tests. Stimulated and unstimulated saliva were collected in plastic vials by spitting method. Level of Mucin 5B was measured by ELISA. Unstimulated saliva flow was significantly lower in OLP patients (P = 0.0001). Stimulated saliva flow was not significantly different between the two groups (P > 0.05). Level of Mucin 5B in unstimulated saliva was significantly lower in OLP group (P = 0.0001) while it was not significantly different in stimulated saliva of the two groups (P > 0.05). Level of Mucin 5B was significantly higher in serum of OLP patients (P = 0.016). Both saliva flow and level of Mucin 5B decrease in OLP patients. Since Mucin 5B is effective for wetting and lubrication of the oral cavity, this result can suggest a possible reason for mouth dryness in OLP patients.

Conflicting results have been reported regarding differing studies on the association between T-cell immunoglobulin and mucin domain 3 polymorphisms and autoimmune disease. The purpose of the present study was to evaluate the association of TIM-3 rs1036199 (4259 G/T) polymorphism with autoimmune disease susceptibility. A meta-analysis was performed to obtain a more precise evaluation of the association. Ten eligible studies were retrieved by searching PubMed, Embase and Web of Science databases, and statistical analyses were performed using STATA software. The pooled results indicated that TIM-3 rs1036199 polymorphism was significantly associated with an increased risk of overall autoimmune disease in allele comparison (G versus T: OR = 1.59, 95%CI: 1.17–2.17) and heterozygous comparison (GT versus TT: OR = 1.68, 95%CI: 1.37–2.06). Subgroup analyses based on disease type demonstrated that TIM-3 rs1036199 polymorphism was associated with an increased risk of rheumatic arthritis (G versus T: OR = 1.88, 95%CI: 1.45–2.44; GT versus TT: OR = 2.02, 95%CI: 1.53–2.65), especially in Asian populations.

Oral Lichen planus (OLP) is a chronic autoimmune cellmediated inflammatory disease. Most OLP patients complain of xerostomia/dry mouth due to changes in the oral mucosal protective barrier. Mucin 5B (MUC5B) is the predominate gelforming mucin in the oral cavity that protects the underlying tissue from mechanical damage, chemical insult, and pathogenic microbes. This study aims to assess MUC5B in OLP subjects and correlate its role in etiopathogenesis. The study included 20 OLP subjects and 10 healthy controls. OLP subjects were divided further into 10 keratotic (reticular) and 10 nonkeratotic (atrophic/erythematous) groups based on the clinical criteria. Dryness of mouth was confirmed by the subjective (Xerostomia Inventory, XI; Bother Index, BI) and clinical testing (Clinical Oral Dryness Score, CODS) method. Stimulated, unstimulated saliva and blood samples were collected from subjects. Samples were centrifuged and analysed for MUC5B level using an ELISA kit. The subjective and clinical testing methods which clinically define xerostomia are correlated with the age and sex of the OLP subjects. The stimulated and unstimulated salivary flow rate of OLP subjects was lower (p<0.05) than controls and was associated with the XI, BI, and CODS (p<0.01) . In contrast, the MUC5B level was increased in the serum of OLP subjects compared to controls (p<0.05) . OLP subjects with xerostomia, displayed lower salivary flow, decrease MUC5B in saliva and increase MUC5B in serum. Alteration of MUC5B in the saliva causes oral mucosal alterations affecting the protection of the oral cavity and increase xerostomia in OLP.
