“…Indeed, clinical and morphological distinction between some entities (e.g., well-differentiated peritoneal mesothelioma or florid peritoneal mesothelial proliferations) may be difficult due to some overlapping morphological and immunohistochemical (IHC) features, including papillary structures, clinical symptoms and imaging findings [ 96 , 97 , 98 , 99 , 100 , 101 , 102 , 103 , 104 , 105 , 106 , 107 , 108 , 109 , 110 , 111 , 112 , 113 ]. This is mainly true if we also consider the new knowledge on the emerging entity of the mesothelioma in situ that can remain hidden for a long time [ 114 , 115 ].…”

“…Thoracentesis and analysis of body fluid cytology is frequently done for pleural effusions of unknown origin. In MPM, diagnosis can be challenging as cytologic features tend to be bland and the tissue invasion that is usually the reliable morphologic criteria for MPM cannot be assessed in a cytology preparation (10). Cytologic diagnosis is also limited to epithelioid subtypes as sarcomatoid subtypes do not shed fluid into the pleural effusions.…”

“…The epithelioid form is more common and related to a better prognosis than the non-epithelial one. Phenotypically, MPM cells are a polygonal or cobblestone-like shape in the epithelioid histotype, while a spindle-like form has been associated with the sarcomatoid MPM type [11]. MPM diagnosis is based on imaging modalities and the evaluation of diagnostic and prognostic biomarkers, such as mesothelin, BRCA-1 associated protein (BAP1), osteo-pontin, fibulin 3 and cytokeratins.…”
