Pleural drainage and pleurodesis: implementation of guidelines in four hospitals

Abstract: The aim of the present study was to evaluate the implementation of the 2003 Dutch guideline on the diagnosis and treatment of malignant pleural effusions, and the potential effect of the implementation on the clinical outcome of pleurodesis. All patients with malignant pleural effusion who had a pleural drain placed with the intention of performing pleurodesis were registered prospectively in four centres. Details of the procedure and fluid recurrence and survival data were noted.Patients with a proven malignan… Show more

“…The American Thoracic Society/European Respiratory Society guideline for the management of MPE recommends that pleurodesis should be limited to patients with pleural fluid pH values greater than 7.30, because of the direct correlation between low pH and poor shortterm survival [15][16][17]. This was not found to be the case in a European study that showed no association between success and LDH and pH [18]. In another Egyptian study that compared the success of different techniques of pleurodesis for malignant pleural effusion, the group subjected to pigtail drainage then pleurodesis had a higher frequency of success than the one subjected to tube thoracotomy drainage followed by pleurodesis [19].…”

“…No treatment exists, and palliative attempts may cause further morbidity and mortality (19,20). MPE was recently reclassified as a separate stage of lung cancer, since it was acknowledged to represent a distinct form of metastatic disease with very short survival (18,21,22).…”

“…(2) Various authors have attempted to determine which pleurodesis technique is the best. Although many well-established guidelines provide general recommendations, (5)(6)(7)(8)(9) there are still a wide variety of practices. (10) The objective of the present study was to determine how pleurodesis has been performed in South and Central America.…”
