Pleomorphic adenoma of the larynx

Abstract: Tumors arising from minor salivary glands are extremely rare neoplasms in the larynx. Of the few reports of pleomorphic adenomas in this site, most have subglottic locations while only one case has involved the true vocal cord and seven cases have had supraglottic locations. We present a case of benign mixed tumor located in the posterior commissure which, to our knowledge, is the first reported in the world literature.

“…The existence of adenocarcinomatous and chondrosarcomatous areas in the lymph node metastasis indicates that this laryngeal malignant mixed tumor originated in the seromucous glands of the larynx. Moreover, this hypothesis is consistent in part with the origin of the malignant mixed tumor in the salivary gland, 21 because some malignant mixed tumors of the salivary gland also arose in a previously pleomorphic adenoma 22 (a so-called benign mixed tumor). The histogenesis of malignant mixed tumor of the larynx is not well established; however, in the major salivary glands, the origin of this tumor has been related to a proliferation of myoepithelial cells, which have been considered as totipotent precursor cells.…”

“…As per the literature, the prognosis of adenoid cystic carcinoma of the submandibular gland is good with early diagnosis, extensive surgical intervention, and postoperative radiotherapy. Adverse prognosis is associated with presentations with an advanced tumor, positive surgical margins, and perineural invasion [ 18 ]. Three of our patients with pleomorphic adenoma of the palate on FNAC had CT features showing cystic components with significant lymph nodes, and the intraop frozen section showed low-grade malignancy.…”

“…A review of the world literature revealed 14 cases involving a pleomorphic adenoma of the larynx 12–15 . But only one case of pleomorphic adenoma of the posterior commissure has been reported 13 . We describe here a rare case of pleomorphic adenoma of the posterior laryngeal commissure with a favorable prognosis.…”
