Introduction: Systemic sclerosis (SSc) or scleroderma is a clinically heterogeneous disease. Autoantibodies associated with different clinical features may help in predicting organ involvement. Complete blood count (CBC) parameters and neutrophil/lymphocyte (NLR), monocyte/lymphocyte (MLR), and platelet/lymphocyte (PLR) ratios, which are considered biomarkers of systemic inflammation, have been reported many times in various rheumatologic diseases. Studies related to the usefulness of the CBC to assess the severity of SSc are still lacking. This study seeks to determine whether CBC parameters associated with organ involvement, when evaluated together with clinical features and autoantibodies, can additionally contribute to risk estimation. Methods: Adult patients with SSc (n = 130) and healthy control (n = 129) groups were enrolled in the study. Epidemiological, clinical, laboratory, and radiological findings were obtained by examining patient records. Results: PLR, NLR, and MLR were related to organ involvement. Statistically significant results were obtained with hemoglobin (≤ 13.0 g/dl), lymphocyte count (≤ 1,900 × 10 3 /ml), and mean platelet volume (≤ 8.0 fl) to estimate the risk of interstitial lung disease (p < 0.05). When the lymphocyte count was 1,400 (10 3 /ml) or less, there was a significantly greater risk of pulmonary hypertension. Neutrophil volume ≤ 141 indicated gastrointestinal tract involvement. Conclusions: Simple hematological parameters can be used for predicting SSc-related organ involvements.

Summary Toll‐like receptors (TLRs) are evolutionarily conserved receptors essential for the host defence against pathogens. Both immune and non‐immune cells can express TLRs, although at different levels. Systemic sclerosis (SSc) is a chronic disease in which autoimmunity, dysregulated profibrotic mediator release and activation of fibroblasts lead to dysregulated collagen deposition and fibrosis. There is now increasing knowledge that the innate immune system and, in particular, TLRs take a part in SSc pathogenesis. The list of endogenous ligands that can stimulate TLRs in SSc is growing: these ligands represent specific danger‐associated molecular patterns (DAMPs), involved either in the initiation or the perpetuation of inflammation, and in the release of factors that sustain the fibrotic process or directly stimulate the cells that produce collagen and the endothelial cells. This review reports evidences concerning TLR signalling involvement in SSc. We report the new DAMPs, as well as the TLR‐linked pathways involved in disease, with emphasis on type I interferon signature in SSc, the role of plasmacytoid dendritic cells (pDCs) and platelets. The dissection of the contribution of all these pathways to disease, and their correlation with the disease status, as well as their values as prognostic tools, can help to plan timely intervention and design new drugs for more appropriate therapeutic strategies.

Systemic sclerosis is the main systemic fibrotic disease with unknown etiology characterized by peripheral microvascular injury, activation of immune system, and wide-spread progressive fibrosis. Microparticles can be derived from any cell type during normal cellular differentiation, senescence, and apoptosis, and also upon cellular activation. Carrying along a broad range of surface cytoplasmic and nuclear molecules of originating cells, microparticles are closely implicated in inflammation, thrombosis, angiogenesis, and immunopathogenesis. Recently, microparticles have been proposed as biomarkers of endothelial injury, which is the primary event in the genesis of tissue fibrosis. Microparticles may have a role in fostering endothelial to mesenchymal transition, thus giving a significant contribution to the development of myofibroblasts, the most important final effectors responsible for tissue fibrosis and fibroproliferative vasculopathy. Thanks to potent profibrotic mediators, such as transforming growth factor beta, platelet-derived growth factor, high mobility group box 1 protein, nicotinamide adenine dinucleotide phosphate oxidase 4, and antifibrotic agents, such as matrix metalloproteinases, microparticles may play an opposite role in fibrosis.
