Plastic Surgery Practice Models and Research Aims under the Patient Protection and Affordable Care Act

Abstract: As the healthcare landscape in the United States changes under the Affordable Care Act (ACA), providers are set to face numerous new challenges. Although concerns about practice sustainability with declining reimbursement have dominated the dialogue, there are more pressing changes to the healthcare funding mechanism as a whole that must be addressed. “…The ACA supports and promotes the concept of bundled payments and formation of ACOs, under which physicians move away from charging separately for each service (fee-for-service model) to instead receiving a ''lump sum'' or ''up-front'' payment for the overall care of patient. [18][19][20] To ensure that hand surgery procedures are adequately reimbursed within this model, highquality data must exist regarding the value and effectiveness of these procedures, something that hand surgeons can actively contribute to. 9,20 Particular emphasis will have to be placed on identifying treatments that not only address patient's health concerns, but also reduce hospital length of stay, save health carerelated costs, and improve long-term outcomes.…”

“…Supporting resident research time is a financial challenge, particularly during a time of declining physician reimbursement, 10 a changing healthcare landscape, 11 and diminishing NIH funding lines. 12,13 To put things into perspective, the mean stipend for a PGY-3/PGY-4 in 2014 was approximately $56,000, 14 and with benefits accounting for an additional 40% of salary, 15 the total annual cost to support a resident's salary during their ADT approaches $80,000.…”

“…44 Although plastic surgeons do not have a favorable impression of the Affordable Care Act, the policy surrounding the Affordable Care Act has had a tremendous impact on the specialty. 45 One aspect of the Affordable Care Act that has become a key driver of the national shift to value-based care is accountable care organizations, which are groups of health care providers that are responsible for the value of care delivered to Medicare beneficiaries. 11,46 In particular, the Medicare Bundled Payments for Care Improvement is a voluntary model that aims to link all payments into one bundle for inpatient stays, outpatient procedures, and any services that are delivered 90 days after discharge.…”
