Plasma Levels of Brain Natriuretic Peptide: A Potential Marker for HIV-Related Cardiomyopathy

Abstract: One of the most common and life-threatening cardiovascular complications of HIV-infection is severe global left ventricular dysfunction due to primary heart muscle disease. At present, there is no single, cost-effective and reliable method of identifying the highly prevalent HIV-related cardiac dysfunction. Nonetheless, growing evidence supports the hypothesis that brain natriuretic peptide measurement has the potential to detect patients who develop HIV-related cardiomyopathy. If true, this hypothesis would h… Show more

“…Elevated levels of BNP have been described in case reports in HIV patients with HIV-related cardiomyopathy [14,20]. In another small study the incidence of myocardial dysfunction in an HIV-infected population receiving state-of-the-art treatment was examined in 91 HIV-infected patients with a radionuclide left and right ventriculography as well as measurement of BNP [16].…”

“…Natriuretic peptides have been studied in a limited number of HIV patients with left-ventricular dysfunction, myocarditis or pulmonary hypertension [1416]. In a sample of participants from the Women's Interagency HIV Study (WIHS), an ongoing, multicenter study of natural history of HIV disease in women, it was shown that HIV-infected women had higher mean NT-pro-BNP levels compared to HIV-uninfected women, but that the differences were attributable to factors such as anemia, kidney disease, and hepatitis co-infection [17].…”

“…Few prior studies have assessed natriuretic peptide levels and cardiac function in HIV-infected populations1215 and none has specifically examined NT-Pro-BNP levels. Carillo-Jimenez found serum levels inversely related to LV function in 10 HIV-infected patients assessed by echocardiography 12.…”

“…Cardiovascular (CV) disease is an increasing concern among patients with HIV infection 8,9. Although natriuretic peptide levels have been proposed to be a neurohumoral marker of CV risk,10,11 few prior studies have evaluated such levels in HIV infection 1215. Accordingly, the objectives of the present study were to identify clinical factors associated with elevated NT-pro-BNP in HIV-infected and -uninfected women and to further investigate the effect of these risk factors on NT-pro-BNP levels in HIV-infected women.…”
