Plasma level of soluble P-selectin in patients with rheumatic mitral stenosis and sinus rhythm undergoing percutaneous mitral balloon valvuloplasty

Abstract: This study suggests that in patients with MS and sinus rhythm sP-selectin level increases probably due to turbulent flow across the stenotic valve. After PMBV sP-selectin shows a progessive decline, which is directly correlated with the increase in MVA.

“…15,17,18 Levels of sPsel are also raised in patients with mitral valve disease but no AF, and such levels fall upon valve repair. 19 Oxidized low-density lipoprotein (oxLDL) may be a mediator of endothelial and platelet perturbation and may serve as a potential prognostic marker for future cardiovascular events. 20 Kim and colleagues 21 reported that lipoprotein properties of patients with AF are more oxidized and modified with smaller particles size compared to controls.…”

“…As in chronic obstructive pulmonary disease, pulmonary hypertension is usually present in MS because of the increased left atrial pressure, pulmonary arteriolar constriction and obliterative changes in the pulmonary vascular bed. Thirdly, Topaloglu et al [16] found high sP-selectin levels in MS patients. Also, it was shown that AVP caused P-selectin release.…”

“…In light of these data, Topaloglu et al [16] evaluated soluble P-selectin (sP-selectin), which is one of the platelet activation markers, in MS patients. This molecule, a component of the membrane of the alpha and dense granules of platelets, is responsible for the adhesion of platelets to leukocytes or the endothelium.…”

“…However, previous studies with a larger sample size have shown significant decrease of left atrial Pselectin level at 24 hour, and 4 weeks after PBMV with no significant difference between peripheral and left atrial P-selectin. 4,5 Hasan-Ali et al showed that P-selectin change correlated with the change in left atrial diameter and pulmonary artery systolic pressure after 2 weeks of PBMV procedure in 65 patients. 5 It may take a longer period of altered hemodynamic to reduce left atrial soluble P-selectin in MS patients following PBMV.…”
