AIM:To evaluate the association between the levels of homocysteine (Hcy), folate, vitamin B12 in human immunodeficiency virus (HIV)-infected patients who were treated with antiretroviral therapy (ART) or not treated with ART. METHODS:The PubMed and Scielo databases were searched. Eligible studies regarding plasma Hcy level in HIV-infected patients were firstly identified. After careful analysis by two independent researches, the identified articles were included in the review according to two outcomes (1) Hcy, folate and vitamin B12 blood concentration in HIV-infected subjects vs health controls and; (2) Hcy blood concentration in HIV-infected subjects under ART vs not treated with ART. RevMan (version 5.2) was employed for data synthesis. RESULTS:A total of 12 studies were included in outcome 1 (1649 participants, 932 cases and 717 controls). Outcome 1 meta-analysis demonstrated higher plasma Hcy (2.05 µmol/L; 95%CI: 0.10 to 4.00, P < 0.01) and decreased plasma folate concentrations (-2.74 ng/mL; 95%CI: -5.18 to -0.29, P < 0.01) in HIV-infected patients compared to healthy controls. No changes in vitamin B12 plasma concentration were observed between groups. All studies included in the outcome 2 meta-analysis (1167 participants; 404 HIVinfected exposed to ART and 757 HIV-infected non-ART patients) demonstrated higher mean Hcy concentration in subjects HIV-infected under ART compared to non-ART HIV subjects (4.13 µmol/L; 95%CI: 1.34 to 6.92, P < 0.01). CONCLUSION:This meta-analysis demonstrated that the levels of Hcy and folate, but not vitamin B12, were associated with HIV infection. In addition, Hcy levels were higher in HIV-infected patients who were under ART compared to HIV-infected patients who were not exposed to ART. Our results suggest that hyperhomocysteinemia should be included among the several important metabolic disturbances that are associated with ART in patients with HIV infection. 147 META-ANALYSIS

Key Points• HIV+ disease progresses with increased oxidative stress and among the defi cient antioxidants is GSH.• GSH levels can be restored in plasma and intracellularly by oral administration of its amino acids precursors.• Presently, we restored HIV+ plasma GSH to normal control values by using Gln as dietary supplement.• The only GSH precursor fully replenished was Gly.• HIV+ and controls responded similarly to the supplementation by increasing plasma Gln but differently for Glu with much less increase in HIV+.• The contribution of Gln to higher Gly could be either due to increased protein breakdown or more probably by decreasing Gly catabolism. • Gly is the major substrate in Ser synthesis in an interconvertible reaction that accounts for almost half of the whole-body Gly fl ux.• Moreover, the Gly clearance through GCS accounts for about 25 % of Gly fl ux leading to both one-carbon (and Hcy synthesis) metabolism and Ser synthesis.• In both situations the higher Gly catabolism would result in higher formation of Hcy and mainly Ser which were not found here.• The present fi ndings of GSH restoration by oral GSH supplementation following the increased Gly and reduced Ser levels seem more likely related to a slower breakdown of Gly resulting in lower Ser formation.

The outbreak of coronavirus disease 2019 (COVID‐19) has caused a worldwide pandemic since 2019. A metabolic disorder is a contributing factor to deaths from COVID‐19. However, the underlying mechanism of metabolic dysfunction in COVID‐19 patients and the potential interventions are not elucidated. Here targeted plasma metabolomic is performed, and the metabolite profiles among healthy controls, and asymptomatic, moderate, and severe COVID‐19 patients are compared. Among the altered metabolites, arachidonic acid and linolenic acid pathway metabolites are profoundly up‐regulated in COVID‐19 patients. Arginine biosynthesis, alanine, aspartate, and glutamate metabolism pathways are significantly disturbed in asymptomatic patients. In the comparison of metabolite variances among the groups, higher levels of l‐citrulline and l‐glutamine are found in asymptomatic carriers and moderate or severe patients at the remission stage. Furthermore, l‐citrulline and l‐glutamine combination therapy is demonstrated to effectively protect mice from coronavirus infection and endotoxin‐induced sepsis, and is observed to efficiently prevent the occurrence of pulmonary fibrosis and central nervous system damage. Collectively, the data reveal the metabolite profile of asymptomatic COVID‐19 patients and propose a potential strategy for COVID‐19 treatment.
