Background: Oral microbiota has been at the center of cultural attention in recent years. In daily clinical practice, orthodontic appliances may be associated with an increased cariogenic risk and a worsening of preexisting periodontal diseases.Objective: The purpose of this review is to investigate the available evidence regarding the association between orthodontic appliances and changes in the quality and quantity of the oral microbiota.Design: The research included every article published up to October 2017 featuring the keywords ‘Orthodontic appliance* AND (microbiological colonization OR periodontal pathogen* OR Streptococcus mutans OR Lactobacillus spp. OR Candida OR Tannerella forsythia OR Treponema denticola OR Fusobacterium nucleatum OR Aggregatibacter actinomycetemcomitans OR Prevotella intermedia OR Prevotella nigrescens OR Porphyromonas gingivalis)’ and was conducted in the major medical databases. The methodological quality of selected papers was scored using the ‘Swedish Council on Technology Assessment in Health Care Criteria for Grading Assessed Studies’ (SBU) method.Results: Orthodontic appliances influence the oral microbiota with an increase in the counts of S. mutans and Lactobacillus spp. and in the percentage of potentially pathogenic gram-negative bacteria.Conclusions: There is moderate/high evidence regarding the association between orthodontic appliances and changes in the oral microbiota.PROSPERO registration number CRD42018091589.

Alterações no volume do fluido crevicular e nos parâmetros periodontais, após a colagem de bráquetes metálicos autoligados e convencionais. CAPÍTULO 2 41Micro-organismos relacionados direta ou indiretamente à cárie dental, na saliva e in situ, em diferentes tipos de bráquetes (convencionais e autoligados). CAPÍTULO 3 55Detecção de complexos microbianos pela técnica checkerboard DNA DNA hybridization, pré e pós-colagem de bráquetes convencionais e autoligados. CAPÍTULO 4 75Níveis de citocinas no fluido crevicular e perfil microbiano in situ, após a colagem de bráquetes convencionais e autoligados. ® R e SmartClip TM em incisivos e caninos superiores. Os índices, o volume do fluido e as amostra de saliva (S0) foram obtidos antes da instalação dos aparelhos (T0) e após 30(T1) e 60(T2) dias. Um bráquete de cada tipo foi removido 30 e 60 dias após a colagem. Os dados foram submetidos à análise estatística utilizando-se os testes não-paramétricos de Friedman, Mann-Whitney e Wilcoxon e coeficiente de correlação não-paramétrico de Spearman. A análise da concentração de citocinas no fluido crevicular foi realizada segundo modelo de análise de variância misto, seguida pelo teste de Tukey, com nível de significância de 5%. Não houve correlação entre o grau de apinhamento, overjet e overbite com os escores do índice de placa (PI), índice gengival, índice de sangramento gengival e com o volume do fluido crevicular no tempo inicial T0 (p>0,05). Verificou-se a diferença significativa nos escores de PI e volume do fluido crevicular somente nos dentes que receberam bráquetes autoligados SmartClip TM , entre os tempos T0-T2(p<0,05). Identificou-se a presença de 21 das 22 espécies bacterianas e 4 das 5 espécies fúngicas, na saliva, antes da colagem dos aparelhos ortodônticos. In situ, no bráquete autoligado SmartClip TM observou-se maiores níveis de espécies do complexo vermelho 60 dias após a colagem. Para os bráquetes convencionais Gemini TM verificou-se diminuição nos níveis de espécies do complexo roxo. Para o bráquete autoligado In-Ovation ® R identificou-se diferença significativa para C. rectus 30 dias após a colagem e para S. mutans 60 dias após a colagem. Quando comparou-se os níveis salivares e os in situ, verificou-se maiores níveis de espécies nos bráquetes, in situ, sendo que os autoligados apresentaram os maiores valores.Aumento significante foi verificado para os bráquetes autoligados In-Ovation ® R, para S. mutans e L. casei. Após 60 dias da colagem do aparelho ortodôntico observou-se um aumento nos níveis de TNF-α (p<0.005), em todos os tipos de bráquetes. ObjectivesMagnetron sputtering was evaluated to equip surfaces of orthodontic elastomeric ligatures with silver and bismuth nanofilms.Material and MethodsAntibacterial properties were evaluated by the adhesion of Streptococcus mutans. Polyurethane‐based elastomeric ligatures were coated with silver and bismuth nanofilms via direct current magnetron sputtering. Surface roughness (Ra) and surface‐free energy (SFE) were assessed. Coated specimens were incubated with S. mutans for 2 h. Adhering bacteria were visualized by Hoechst staining and quantified by an ATP‐based luminescence assay. One‐way analysis of variance with Tukey post hoc testing and Pearson correlation analysis were performed (p < .05) to relate bacterial adhesion to surface roughness and surface‐free energy.ResultsElastomeric ligatures were successfully coated with silver and bismuth nanofilms. Ra was significantly reduced by silver coating. Silver and bismuth coatings showed significantly higher SFE than controls. Adhesion of S. mutans was significantly decreased by silver coating. No correlation between bacterial adhesion and SFE was found. Correlation between bacterial adhesion and Ra was positive but not statistically significant.ConclusionsMagnetron sputtering proved to be a feasible method to equip orthodontic elastomeric ligatures with silver and bismuth nanofilms. Silver coatings of elastomeric ligatures may reduce white spots and carious lesions in orthodontic patients. Future research is required to stabilize coatings.
