Pituitary abscess

Abstract: All cases of pituitary abscesses (19 patients) reported in the literature since 1970 are reviewed. Based on the data available, it is suggested that a pituitary abscess is not a bacterial infection as commonly assumed. Probably, it represents a normal tissue reaction to an infarction of a pituitary adenoma. The frequent association with negative cultures from the abscesses, previous sterile meningitis and endocrine disturbances, and concurrent rhinorrhoea are well explained in this way.

“…Controversy exists concerning the confirmation of PA in sterile lesions. Some authors consider sterile abscesses as sequelae of the aseptic necrosis of pituitary tumors [17]. The management recommended for PA is surgical drainage by the transsphenoidal approach and antibiotic treatment.…”

“…[4][5][6][7]18) Pituitary abscess tends to occur after surgery for a tumor in the pituitary region, 17) and sometimes occurs in pituitary adenoma, 12,17) Rathke's cleft cyst, 3,10,13) and craniopharyngioma. 14) The lesion may be localized in the pituitary regions because degradation of immunocompetence, presence of necrotic tissue, 2) and complicating hypopituitarism contribute to reduced resistance to infection. 5,7,9) Direct infection from the surrounding tissue derived from meningitis, 8,10,17) sphenoid sinusitis, 16) tooth extraction, 17) cavernous sinus thrombosis, sepsis, 17) and immunocompromised disease 11) can also cause pituitary abscess.…”
