Blurred Vision Caused by Inflammation of the Optic Nerves Due to a Pituitary Abscess-Case Report-

Abstract: A 48-year-old woman was admitted to our institution complaining of headache and a 2-month history of blurred vision and increased urinary volume. T 1 -weighted magnetic resonance (MR) imaging with gadolinium showed a ring-enhanced mass lesion in the pituitary. Fluid-attenuated inversion recovery MR imaging showed high-intensity lesions in the optic nerves, hypothalamus, and thalamus. The histological diagnosis was pituitary abscess. “…The presence of a round cystic mass with a thin peripheral rim enhancement following gadolinium injection at MRI, occasionally associated with a thickened pituitary stalk and symptoms of diabetes insipidus may be suggestive of pituitary abscess. Based on the MRI image, differential diagnosis of the sellar mass with other diseases such as a pituitary adenoma [2,7,32], pituitary apoplexy [33], pituitary germinal tumor [34] or hypophysitis [3] is to be considered. Diabetes insipidus has been reported only in some cases, but pituitary insufficiency and symptoms of intracranial mass are common; in few cases, pituitary insufficiency may not develop as shown by Takahashi in a 13-years-old girl with pituitary abscess associated to mild sphenoid sinusitis [7].…”

“…Pituitary abscess is a very rare condition, comprising only around 0.3 % of pituitary tumours and less than 1 % of all pituitary diseases ( Domingue, Wilson 1977 ;Vates et al, 2001 ; Bakker and Hoving, 2010 ). Symptoms are similar to other pituitary tumours including headache, visual fi eld defects and problems associated with pituitary hormone defi ciency ( Utsuki et al, 2005 ;Carpinteri et al, 2009 ).
