Pigmented villonodular synovitis: MRI characteristics

Abstract: The magnetic resonance imaging (MRI) scans of 26 patients with histopathologically proven pigmented villonodular synovitis (PVNS), involving joints but excluding tendon sheaths, were reviewed retrospectively. The purpose of this study is to define the spectrum and frequency of MRI characteristics for PVNS using conventional spin echo (in two cases before and after intravenous administration of gadopentate dimeglumine) and also gradient echo techniques. A cystic variety is presented, the MRI appearances of whic… Show more

“…T 2 -weighted MR imaging of GCRG, GCT, and PVNS demonstrates extremely low intensity, 10,11,14,18,19,24,34,35) whereas the other tumors usually appear as high intensity. We consider that this finding is very important in the differential diagnosis.…”

“…Previous terminology for PVNS included synovial xanthoma, synovial fibroendothelioma or endothelioma, benign fibrous histiocytoma, xanthomatous giant cell tumor of the tendon sheath, myeloplaxoma, chronic hemorrhagic villous synovitis, giant cell fibrohemangioma, fibrohemosideric sarcoma, sarcoma fusigiganocellulare, benign or malignant polymorphocellulartumor of the synovial membrane, and fibrous xanthoma of the synovial membrane. [11][12][13][14][15][16] The imaging findings of PVNS include: PLAIN RADIOGRAPH:Radiographic features are nonspecific, and radiographs may appear normal. The diffuse intra-articular form of PVNS oftendemonstrates a joint effusion and extrinsic erosionof bone on both sides of the joint, but thejoint space is unaffected. The localized formsof disease usually reveal only a soft-tissue mass.…”

“…Associated large joint effusion is common, but the cartilage is preserved and subchondral erosions may be seen only in late phases. Although, the defi nitive diagnosis is made by biopsy, the MR imaging fi ndings are virtually pathognomonic for pigmented villonodular synovitis [ 76 ].…”
