Bilateral salpingo-oophorectomy (BSO) has become the standard of care for risk reduction in women at hereditary risk of ovarian cancer. While this procedure significantly decreases both the incidence of and mortality from ovarian cancer, it impacts quality of life, and the premature cessation of ovarian function may have long term health hazards. Recent advances in our understanding of the molecular pathways of ovarian cancer point to the fallopian tube epithelium as the origin of most high grade serous cancers (HGSC). This evolving appreciation of the role of the fallopian tube in HGSC has led to the consideration of salpingectomy alone as an option for risk management, especially in premenopausal women. In addition, it is postulated that bilateral salpingectomy with ovarian retention (BSOR), may have a public health benefit for women undergoing benign gynecologic surgery. In this review we provide the rationale for salpingectomy as an ovarian cancer risk reduction strategy.

ObjectiveRecent evidence has supported the concept that epithelial ovarian cancer (EOC) arises from the cells of the fallopian tube or endometrium. This study investigated current practice in Japan with respect to performing opportunistic bilateral salpingectomy (OBS) during gynecological surgery for benign disease for Ovarian Cancer Prevention.MethodsWe mailed a questionnaire to 767 hospitals and clinics, comprising 628 accredited training institutions of the Japan Society of Obstetrics and Gynecology (JSOG), Japan Society of Gynecologic Oncology (JSGO), or Japan Society of Gynecologic and Obstetric Endoscopy and Minimally Invasive Therapy (JSGOE) and 139 private institutions with at least one JSGOE-certified licensed gynecologic laparoscopist.ResultsAmong the 767 institutions, 444 (57.9%) provided responses, including 91 (20.6%) that were both JSGOE and JSGO accredited, 71 (16.0%) that were only JSGO accredited, 88 (19.8%) that were only JSGOE accredited, and 194 (43.7%) that were unaccredited. It was found that awareness and performance of OBS largely depended on the JSGO and/or JSGOE accreditation status. OBS was only performed at 54.0% of responding institutions and just 6.8% of the institutions were willing to participate in randomized controlled trials to validate this method for reducing the incidence of ovarian cancer.ConclusionThe JSOG Gynecologic Tumor Committee will announce its opinion on salpingectomy for ovarian cancer prevention to all JSOG members and will develop a system for monitoring the number of OBS procedures in Japan.

12В структуре онкологической патологии у женщин рак яичников является одним из часто встречающихся заболеваний, при этом обладая наихудшим прогнозом. Рак яичников имеет один из самых высоких показателей смертности несмотря на то, что его доля составляет всего 3% всех онкологических заболеваний у женщин. Ежегод-но в мире регистрируется около 140 000 смертей от рака яичников и примерно у 225 000 женщин впервые диагно-стируют это заболевание [1]. Традиционные представле-ния о патогенезе сводились к мнению, что рак яичника возникает из эпителия, покрывающего этот орган, или его эпителиальных включений. В связи с этим все иссле-дования канцерогенеза яичника были сконцентрированы на его эпителии, однако никаких предвестников данной патологии обнаружено не было. Относительно недавно была выдвинута иная теория, которая полагает, что наи-более часто встречающийся вид злокачественных опухо-лей яичника (низкодифференцированная серозная кар-цинома) возникает из низкодифференцированной интра-эпителиальной серозной карциномы в маточной трубе, которая затем распространяется на поверхность яичника и брюшины (рис. 1).Маточная труба редко подвергалась тщательному ги-стологическому исследованию, так как традиционно рас- Цель -улучшение отдаленных результатов лапароскопической гистерэктомии с двусторонней тубэктомией в репродук-тивном периоде. Материал и методы. В исследование были включены 54 пациентки, которым после рандомизации лапа-роскопическим доступом была выполнена гистерэктомия с сохранением яичников в сочетании с тубэктомией и без нее. В 1-ю группу вошли 29 пациенток с тубэктомией, 2-ю группу составили 25 пациенток без тубэктомии. На догоспитальном этапе и спустя 3-4 мес после операции пациенткам проводили обследование на уровень АМГ, лГ, ФСГ, эстрадиола и тестостерона, ультразвуковое исследование органов малого таза, анкетирование опросником SF-36 и визуальной аналого-вой шкалой оценки боли. Результаты. В обеих группах не было выявлено статистической разницы в показателях возраста пациенток, индекса массы тела, продолжительности операции, кровопотери, длительности пребывания в стационаре, в показателях уровня гормонов, в объеме яичниковой ткани и показателях кровотока яичников. В обеих группах пациенток отмечено улучшение как физического, так и ментального здоровья. Заключение. Приведенные данные могут свидетель-ствовать о безопасности тубэктомии в отношении функции яичников.Ключевые слова: лапароскопия, гистерэктомия, профилактическая тубэктомия, профилактическая сальпингэктомия, функция яичников, клиническое исследование.
