ResumoObjetivo: Avaliar se a violência doméstica na gestação está associada a desfechos desfavoráveis na saúde do lactente, medidos pelo baixo peso ao nascer ou prematuridade.Método: Estudo de coorte prospectiva, realizado com gestantes que fizeram pré-natal em 10 Unidades Básicas de Saúde do município de Campinas (SP), durante os anos de 2004 a 2006. Foi utilizado questionário estruturado e validado no Brasil. As gestantes tiveram, no mínimo, duas e, no máximo, três entrevistas realizadas durante pré e pós-natal. Foi utilizada análise descritiva dos dados. O teste t de Student foi utilizado para comparar as médias do peso ao nascer e da idade gestacional entre os grupos de gestantes que sofreram, durante a atual gestação, violência doméstica e aqueles que não sofreram. A análise de regressão logística foi utilizada para verificar os fatores associados ao baixo peso ou prematuridade. Resultados:Foram acompanhadas durante o período de pré-natal e pós-natal (n = 1.229) 89,1% das gestantes; 10,9% representam as perdas de acompanhamento, basicamente por mudança de endereço. O peso médio ao nascer foi de 3.233 g; idade gestacional foi em média 38,56 semanas. Apresentaram baixo peso ao nascer ou prematuridade 13,8% dos recém-nascidos. Condições de risco para baixo peso ao nascer ou prematuridade foram: gestante ter tido recém-nascido prematuro em outra gestação (p < 0,005), ser tabagista (p < 0,001), ter tido parto por cesárea (p < 0,001), ser baixa a escolaridade do parceiro (p < 0,008). Conclusão:Neste estudo, não foi observada associação estatisticamente significativa entre violência doméstica perpetrada pelo parceiro e baixo peso ao nascer ou prematuridade. Methods:This was a prospective cohort study enrolling pregnant women whose prenatal care was provided by 10 basic health units in the city of Campinas, SP, Brazil, between 2004 and 2006. A structured questionnaire was employed that had previously been validated for use in Brazil. Each mother attended a minimum of two and a maximum of three interviews during the prenatal and postnatal periods. Data were analyzed using descriptive statistics. Student's t test was used to compare means for birth weight and gestational age between mothers who had suffered domestic violence during the current pregnancy and those who had not. Objective-Little is known about how older African American women define family violence (FV) and what FV survivors might expect from their healthcare providers. The purpose of this study was to understand how these women define FV, where they seek help for FV, and what barriers they face in these efforts.Methods-We conducted 6 focus groups with 30 African American women over the age of 50, including some FV survivors, at a large, inner-city public hospital.Results-Participants defined FV broadly, citing examples of abuse (physical, sexual, emotional and financial) and neglect. Spiritual sources were cited over physicians as being available to help FV survivors. Barriers to receiving assistance included negative encounters with physicians, lack of trust in the system and dearth of age-appropriate resources.Conclusions-For older African American women, FV takes many forms of which many may not be obvious during the clinical encounter. Like younger FV survivors, they expect physicians to serve as a resource for FV.Practice implications-Physicians caring for older African American women need to remember to ask them about FV, and when making referrals for abuse and neglect, consider offering referrals to pastoral care if appropriate.

Adolescent Relationship Violence: Help-Seeking and Help-Giving Behaviors among Peers

Young people tend to disclose relationship violence experiences to their peers, if they disclose at all, yet little is known about the nature and frequency of adolescent helpseeking and help-giving behaviors. Conducted within a sample of 1,312 young people from four New York City high schools, this is the first paper to ask adolescent help-givers about the various forms of help they provide and among the first to examine how ethnicity and nativity impact help-seeking behaviors. Relationship violence victims who had ever disclosed (61 %) were more likely to choose their friends for informal support. Ethnicity was predictive of adolescent disclosure outlets, whereas gender and nativity were not. Latinos were significantly less likely than non-Latinos to ever disclose to only friends, as compared to disclosing to at least one adult. The likelihood of a young person giving help to their friend in a violent relationship is associated with gender, ethnicity, and nativity, with males being significantly less likely than females to give all forms of help to their friends (talking to their friends about the violence, suggesting options, and taking action). Foreign-born adolescents are less likely to talk or suggest options to friends in violent relationships. This study also found that Latinos were significantly more likely than non-Latinos to report taking action with or on behalf of a friend in a violent relationship. This research shows that adolescents often rely on each other to address relationship violence, underlining the importance of adolescents' receipt of training and education on how to support their friends, including when to seek help from more formal services.
