Physical conditions and challenging behaviour in people with intellectual disability: a systematic review

Abstract: Medical conditions can play a role in challenging behaviour, and this should be evaluated in the clinical setting. So far, the level of evidence is generally low, and longitudinal studies are completely lacking. We recommend a systematic approach to research examining the role of physical conditions in challenging behaviour, the ultimate aim being to establish a basis for the development of clinical guidelines.

“…Epilepsy is another medical condition that may have a role in SIB, as patients with temporal lobe seizures can have associated irritability or aggression, and several case studies have reported improved SIB after treatment of seizures (Bear et al 1982;Blumer 2000;Davanzo and King 1996;Shakya et al 2010). Several recent studies have found no increased prevalence of aggression between groups with and without epilepsy (de Winter et al 2011;Matson et al 1999;Matthews et al 2008). However, a subgroup of individuals may exist that have epilepsy and SIB.…”

“…11 Notwithstanding, it does specifically assess behavioral repertoires associated with aggression, selfinjury, and stereotyped behaviors often observed in people with intellectual disability and requiring close monitoring. 14 The availability of standardized instruments focusing on these behavior problems could help identify and treat these problems. In contrast, the absence of such instruments poses difficulties to many healthcare and education professionals.…”
