The sick preterm infant monitoring is an intriguing job that medical staff in Neonatal Intensive Care Units (NICU) must deal with on a daily basis. As a standards monitoring procedure, preterm infants are monitored via sensors and electrodes that are firmly attached to their fragile and delicate skin and connected to processing monitors. However, an alternative exists in contactless imaging to record such physiological signals (we call it as Physio-Markers), detecting superficial changes and internal structures activities which can be used independently of, or aligned with, conventional monitors. Countless advantages can be gained from unobtrusive monitoring not limited to: (1) quick data generation; (2) decreasing physical and direct contact with skin, which reduces skin breakdown and minimizes risk of infection; and (3) reduction of electrodes and probes connected to clinical monitors and attached to the skin, which allows greater body surface-area for better care. This review is an attempt to build a solid ground for and to provide a clear perspective of the potential clinical applications of technologies inside NICUs that use contactless imaging modalities such as Visible Light Imaging (VLI), Near Infrared Spectroscopy (NIRS), and Infrared Thermography (IRT).

Cardiovascular disease (CVD) is a widespread disease and the leading cause of death worldwide. Home care is essential for patients with CVD, and it involves the daily monitoring of important CVD-related vital signs using methods including electrocardiography (ECG), heart rate monitoring, pulse oximetry (SpO2), and continuous blood pressure measurement. However, a wearable device that can monitor these parameters simultaneously remains unavailable; herein, we propose a lightweight, highly integrated sensor that can do so. In this sensor, an analog front end (AFE) integrated chip (IC) is implemented in the sensor to detect one-lead ECG and two-wavelength photoplethysmography (PPG) signals. The highly integrated IC minimizes both the size and power requirement of the sensor. Moreover, its comprehensive functions include adjustable gain, current compensation, lead off, and fast recovery-all of which are crucial for wearable applications in large populations. Accordingly, the sensor can apply the adaptive adjust method to automatically adjust the IC parameters to suit a range of applications and users. In addition, the heart rate is calculated from the R-R interval from the ECG signals, whereas the SpO2 is calibrated with a univariate quadratic equation with less than 1% mean error. Our sensor can also calculate the systolic blood pressure (SBP) and diastolic blood pressure (DBP) by using a support vector machine based calibration-less model with the features of infrared PPG and ECG signals. The model is trained on our pre-collected wearable dataset and has a mean error ± standard deviation of −2.10 ± 7.07 mmHg for SBP and 0.04 ± 7.34 mmHg for DBP in 16 volunteers. In conclusion, this paper reports on a multimodal, vital sign monitoring sensor-with small size, low power, and dynamic compatibility-suitable for patients with CVD under home care.

The task of heart rate estimation using photoplethysmographic (PPG) signal is challenging due to the presence of various motion artifacts in the recorded signals. In this paper, a fast algorithm for heart rate estimation based on modified SPEctral subtraction scheme utilizing Composite Motion Artifacts Reference generation (SPECMAR) is proposed using two-channel PPG and three-axis accelerometer signals. First, the preliminary noise reduction is obtained by filtering unwanted frequency components from the recorded signals. Next, a composite motion artifacts reference generation method is developed to be employed in the proposed SPECMAR algorithm for motion artifacts reduction. The heart rate is then computed from the noise and motion artifacts reduced PPG signal. Finally, a heart rate tracking algorithm is proposed considering neighboring estimates. The performance of the SPECMAR algorithm has been tested on publicly available PPG database. The average heart rate estimation error is found to be 2.09 BPM on 23 recordings. The Pearson correlation is 0.9907. Due to low computational complexity, the method is faster than the comparing methods. The low estimation error, smooth and fast heart rate tracking makes SPECMAR an ideal choice to be implemented in wearable devices.
