The main goal of the present work was to investigate the damages photoinduced by pterin (Ptr), an endogenous photosensitizer present in human skin under pathological conditions, on a globular protein such as ubiquitin (Ub). A particular attention has been paid on the formation of covalent adducts between Ptr and the protein that can behave as photoantigen and provoke an immune system response. Here, a multifaceted approach including UV/visible spectrophotometry, fluorescence spectroscopy, electrophoresis, size exclusion chromatography and mass spectrometry is used to establish the Ub changes triggered by UVA irradiation in the presence of Ptr. Under anaerobic conditions, the only reaction corresponds to the formation of a covalently bound Ptr-Ub adduct that retains the spectroscopic properties of the free photosensitizer. A more complex scheme is observed in air-equilibrated solutions with the occurrence of three different processes ie. formation of a Ptr-Ub adduct, dimerization and fragmentation of the protein. General Chemicals. Pterin (Ptr, purity > 99%, Schircks Laboratories, Switzerland) and ubiquitin from bovine erythrocytes, whose sequence is identical to the human protein, (Sigma-Aldrich, St. Louis, MO) were used without further purification after checking for impurities by HPLC. Sodium dodecyl sulfate (SDS, ~ 99 %), glycerol, 2mercaptoethanol, bromophenol blue, glycine (Gly, > 99% titration), ammonium persulphate (> 98%), N,N,N´,N´-tetramethylethylene-diamine (TEMED, ~ 99 %), Cytochrome c (Cyt) from horse heart, E-3,5-dimethoxy-4-hydroxycinnamic acid (E-SA), α-cyano-4-hydroxycinnamic acid (CHCA), bradykinin (1-7), insulin, angiotensin I and II and guanidine chloride were provided by Sigma Aldrich. Methanol was provided by Laboratorios Cicarelli. Trifluoroacetic acid (TFA) was provided by Merck. Acetic acid was provided by Anedra. Coomassie Brilliant Blue G was provided by Fluka. Acrylamide, N´,N´-methylene-bis-acrylamide and trishydroxymethylaminomethane (Tris) were purchased by Genbiotech. NaCl and acetonitrile were provided by J. T. Baker. Aqueous solutions were prepared using ultrapure water from Milli-Q ® purification system (Millipore Corporation, USA). Measurements of pH. The pH measurements were performed using a pH-meter sensION+ pH31 GLP combined with a pH electrode 5010T (Hach) or microelectrode XC161 (Radiometer Analytical). The pH of the aqueous solutions was adjusted by adding drops of HCl and NaOH solutions from a micropipette. The concentration of the acid and the base used for this purpose ranged from 0.1 to 2 M. UV⁄vis analysis. Electronic absorption spectra were recorded on a Shimadzu UV-1800 spectrophotometer. Measurements were made using quartz cells of 0.4 or 1 cm optical pathlength. The absorption spectra of the solutions were recorded at regular intervals of irradiation time.

SUMMARY -When taking diff erent drugs, their possible side eff ects on the skin should be considered, including skin reactions connected to photosensitivity. Th is photosensitivity caused by drugs can appear as phototoxic reactions (which occur more often) or photoallergic reactions (which occur less often and include allergic mechanisms). Th e following drugs stand out as medications with a high photosensitivity potential: nonsteroidal anti-infl ammatory drugs (NSAIDs), cardiovascular drugs (such as amiodarone), phenothiazines (especially chlorpromazine), retinoids, antibiotics (sulfonamides, tetracyclines, especially demeclocycline and quinolones), etc. In recent years, photosensitive reactions to newer drugs have appeared, e.g., targeted anticancer therapies such as BRAF kinase inhibitors (vemurafenib, dabrafenib), EGFR inhibitors, VEGFR inhibitors, MEK inhibitors, Bcr-Abl tyrosine kinase inhibitors, etc. In patients taking drugs over a longer period of time (e.g., NSAIDs, cardiovascular drugs, etc.), a particular problem arises when an unrecognized drug-induced photosensitivity on the skin manifests in summer months. When taking patient histories, the physician/dermatovenereologist should bear in mind that any drug the patient is currently taking may be the cause of skin reactions. Th erefore, patients who use potentially photosensitive drugs and treatments on a long term basis should be warned of the possibility of these side eff ects on their skin and advised to avoid direct exposure to sunlight and to use adequate photoprotection. If patients carefully protect themselves from the sun, it is often not necessary to stop treatments that include photosensitive drugs. If such reactions appear, anti-infl ammatory and antiallergic therapies should be introduced.

Drug-induced photosensitivity reactions are significant adverse effects. Ketoprofen is one of the most common drugs that can cause skin rash in sun-exposed areas. Non-steroidal anti-inflammatory drugs (NSAIDs), such as ketoprofen, are often used for a variety of symptoms, including pain and fever. An understanding of the presentation and clinical course of ketoprofen-induced photosensitivity is necessary to correctly diagnose and manage this condition. Ketoprofen-induced photosensitivity reactions usually present as photoallergic dermatitis, which is a cell-mediated immune process. The benzophenone moiety in ketoprofen plays a major role in ketoprofen's ability to act as a photosensitizer. Several agents, such as fenofibrate and octocrylene have been found to be associated with aggravation of ketoprofen-induced photoallergic dermatitis or cross-photosensitization, and these reactions result from structural similarities with ketoprofen. Treatment of ketoprofen-induced photoallergic dermatitis includes discontinuation of ketoprofen, topical or systemic corticosteroids and avoidance of sun exposure and agents known to exacerbate dermatitis. In conclusion, photoallergic dermatitis is a significant adverse effect of ketoprofen. Some agents known to worsen dermatitis may be found in sun protection products (notably, octocrylene in sunscreen). Educating the patient to avoid these products is critical to treatment. Since NSAIDs, such as ketoprofen, are used commonly for a variety of illnesses, drug-induced photoallergic dermatitis should be high on the differential in individuals using these medications who present with acute onset of a rash in sun-exposed areas.
