Aim. In this study, tissue harmonic imaging (THI) was compared with conventional gray scale ultrasonography (CUS) by scoring system in measurement of nuchal translucency (NT) and in examination of fetal posterior neck region. Additionally, it was also investigated that whether body habitus of patient provide effect on this condition or not. Methods. Fetal neck region and nuchal translucency were evaluated by using scoring system (1: not seen, 2: seen uncertainly, 3: seen acceptably, 4: well seen, and 5: very well seen). The tissue harmonic imaging was compared with B-mode. Results. In normal patients, overall average score (11+0 to 13+6 weeks) for CUS was 3.27 and it was 3.69 for THI. However, in obese patients, overall average score for CUS was 2.96 and it was 3.48 for THI. For the group with normal BMI; examination with tissue harmonic imaging was on average more clearly visible in 44.6% of the cases, the same as in the conventional B-mode in 53%, and worse in only 2.4%. However, in obese group; examination with tissue harmonic imaging was on average more clearly visible in 51.2% of the cases, the same as in the conventional B-mode in 46.4%, and worse in only 2.4%. Conclusion. When normal group and obese groups are completely considered, it was found that THI provides similar or better image quality almost in all of the patients than that of CUS. Bu çalışmada, fetus boyun posterior bölgesinin değerlendirilmesinde ense kalınlığı ölçümü doku harmonik görüntüleme (DHG) ve konvansiyonel gri skala ultrasonografide (KUS) skorlama sistemi ile karşılaştırılmıştır. Ayrıca hastaların vücut habitusunun ölçümlere etkisinin olup olmadığı araştırılmıştır. Yöntem. Fetal boyun bölgesi ve ense kalınlığı skorlama sistemi kullanılarak değerlendirildi (1: görülemeyen, 2: belirsiz görülen, 3: kabul edilebilir ölçüde görülen, 4: iyi görülen, 5: çok iyi görülen). Doku harmonik görüntüleme B mod ultrasonografi ile karşılaştırıldı. Bulgular. Normal hastalarda, genel ortalama puan (11+0 ile 13+6 hafta) KUS'de 3,27 ve DHG ile 3,69 idi. Ancak, obez hastalarda genel ortalama puan KUS' de 2,96 ve DHG ile 3,48 idi. Normal vücut kitle indeksi olan grupta; DHG ile değerlendirmede ortalama olarak çok net görülebilen grup hastaların %44,6'sı, KUS ile hastaların %53'üdür. Sadece %2,4 görüntü kötü sınıfında idi. Ancak obez grupta; DHG ile ortalama olarak çok net görülebilen grup hastaların % 51,2' si, KUS ile hastaların %46,4' ü dür. Sadece %2,4 görüntü kötü sınıfında idi. Sonuç. Purpose. To compare phase inversion harmonic imaging (PIHI) with fundamental imaging (FI) in the evaluation of renal calculi.Methods. Thirty adult patients with renal calculi (17 men, 13 women; mean age 44 years [range, 25-71]) underwent transabdominal sonographic examination of the urinary system. Both kidneys and renal calculi were examined with PIHI and FI. Overall renal conspicuity, calculus visibility, and clarity of posterior shadowing were assessed subjectively using a 4-point scale (0 being the worst, 3 being the best). The maximum diameter of the calculi was measured using both techniques. The effect of body mass index on qualitative scoring and quantitative measurements was evaluated.Results. PIHI improved overall renal conspicuity compared with FI (p < 0.001). The visibility of the calculi and clarity of posterior shadowing were significantly better with PIHI than with FI (p < 0.001 for both parameters). The maximum diameter of calculi was larger with PIHI than with FI (p < 0.001). The superiority of PIHI over FI regarding overall conspicuity of the kidney, visibility of the calculus, and clarity of posterior shadowing scores increased in the obese group (p < 0.001 for all 3 parameters). The mean calculus diameter difference between the 2 techniques was significantly higher in the obese group.Conclusion. The routine use of PIHI is recommended in the evaluation of renal calculi. Focal hepatic lesions characterisation by different sonographic techniques: a prospective analysis

Introduction Ultrasound is usually the first diagnostic investigation for the assessment of liver lesions. Apart from conventional sonography (CS), new grey-scale sonographic techniques have been developed which have increased the application of ultrasound in liver imaging. The present study was undertaken to compare image quality of CS, real-time compound sonography (RTCS), tissue harmonic sonography (THS) and tissue harmonic compound sonography (THCS) in focal liver lesions. Materials and methods 100 patients with focal hepatic lesions were enroled. Lesions were divided into solid and cystic group. Solid lesions were evaluated for lesion conspicuity and elimination of artefacts. For cystic lesions, lesion conspicuity, posterior acoustic enhancement and internal echoes within the lesion were evaluated. Grading was done using 3-5-point scales. Overall image quality was assessed depending on the total points. Results 78 solid and 22 cystic liver lesions were included. THCS showed superior results for lesion conspicuity, elimination of artefacts and overall image quality in solid lesions. RTCS showed similar results as THCS for lesion conspicuity and overall image quality in solid lesions. THS gave better results in cystic lesions for all imaging parameters. Results of THCS though slightly inferior, showed no significant difference from THS, in cystic lesions. CS was found to have least diagnostic value in characterisation. Conclusions For evaluation of focal hepatic lesions, a combination of compound and harmonic sonography, i.e. THCS, is the preferred sonographic technique.Keywords Ultrasonography Á Liver Á Liver lesions Á Tissue harmonic imaging Á Compound US Riassunto Introduzione l'ecografia è di solito l'indagine radiologica iniziale, non invasiva e semplice, per la valutazione delle lesioni epatiche. Oltre alla tradizionale ecografia in scala di grigi (CS), sono state sviluppate delle nuove tecniche, che hanno aumentato le applicazioni dell'ecografia nell'imaging del fegato. Il presente studio è stato intrapreso per confrontare la qualità delle immagini in: CS, ecografia in tempo reale con compound (RTCS), armonica tissutale (THS) e armonica tissutale con compound (THC) nello studio delle lesioni focali epatiche. Materiali e Metodi sono stati arruolati 100 pazienti con lesioni focali epatiche. Le lesioni sono state divise in solide e cistiche. Per le lesioni solide sono stati valutati la visibilità e l'eliminazione di artefatti. Per le lesioni cistica sono stati valutati la visibilità, il rinforzo acustico posteriore e gli echi all'interno delle lesioni. La classificazione è stata effettuata utilizzando una scala da 3 a 5. La qualità generale dell'immagine è stata valutata in base al totale dei punti. Risultati sono state incluse 78 lesioni epatiche solide e 22 cistiche. La THC ha mostrato risultati superiori per la visibilità delle lesioni, l'eliminazione di artefatti e la qualità complessiva dell'immagine delle lesioni solide.
