“…However most of the mentioned instrument wasn't widely available. (9) Iris retractor was initially used to stabilize lens on patient with pseudoexfoliatife syndrome. Phacomeulsification with use of iris retractor was reported safe procedure to be done in subluxated lens.…”

“…Combination of 4-5 iris hooks and CTR ensures centration of Phacoemulsification with combination of CTR and Iris Hook in Subluxated Cataract 10 capsular bag and capsular expansion in 360 O that provides "in the bag" IOL implantation option. (9,10,11) The implantation of CTR immediately after capsulorexis is completed ensures higher safety of remaining procedures because of equal distribution of force by the ring and stabilization of the capsular bag. However the removal of lens material that trapped under capsular ring became difficult and sometimes retained some lens material that stuck.…”

“…An auxiliary iris hook [2][3][4][5] or capsular hook is used in surgical repair of crystalline lens dislocation or subluxation. 7 The iris hook was the earliest internationally applied tool that was used in PHACO to expand the size of the pupil 8 and divide it to create flexible 9 and rigid iris retractors. 10 The earliest iris hook was developed by Hohwig 11 and was applied to treatment of lens subluxation by Merriam and Zheng.…”

“…Iris hooks can be valuable not only in small pupil cases but also in the setting of iris prolapse, IFIS, iridoschisis, and even for capsular support in cases with zonular dehiscence or subluxed crystalline lens [14–18].…”

“…Nevertheless, the threshold of pupil size to perform phacoemulsification without seeking for further dilation with mechanical dilations devices is subjective and can vary based on the level of the surgeon's experience. However, pupil expansion devices can be very valuable not only in small pupil cases but also in the setting of iris prolapse, intraoperative floppy iris syndrome (IFIS), iridoschisis, and for capsular support in cases with zonular dehiscence or subluxed crystalline lens [14][15][16][17][18].…”
