Personalized Reconstruction of Genital Defects in Complicated Wounds with Vertical Rectus Abdominis Myocutaneous Flaps including Urethral Neo-Orifice

Abstract: Non-healing extensive wounds in the perineal region can lead to severe soft tissue infections and disastrous complications, which are not manageable with conservative measures. Specifically in recurrent or advanced pelvic malignancies, irradiation often leads to extensive scarring and wound breakdown, resulting in significant soft tissue defects during surgical tumor excision. Among several surgical options to reconstruct the perineum, the transpelvic vertical rectus abdominis myocutaneous (VRAM) flap has prov… Show more

“…There are several potential explanations for the high rate of wound infections. Firstly, the pubic region is near the opening of the urinary and gastrointestinal tract, facilitating microbial seeding from these bacteria-laden areas and increasing the risk for infection [ 20 ]. Secondly, the resection of type III tumors without proper reconstruction results in large dead spaces that will become filled with static blood, providing a favorable environment for bacterial growth.…”

“…In addition to combined two-step operations with a pretreatment of the wounds by allografting, composite grafts constructed in vitro have been used experimentally and clinically [ 29 ]. They are a crucial therapeutic option for large-scale wounds that cannot be repaired with tissue flaps and instead rely on skin transplants [ 30 , 31 ].…”

“…In this study, the patient population was retrospectively reviewed and compared by subgroup analysis concerning the reconstructive procedure, the use of negative-pressure wound therapy, and bacterial wound burden and recurrence rate amongst others. Other than in irradiated sites where flaps are a prerequisite for permanent closure, skin grafts do have an important place in the treatment of HS (24)(25)(26)(27) Many patients suffer from a chronic and exhausting course including ineffective therapy efforts with multiple interventions. Single minor lesions can be treated by direct Continuous data are expressed as mean ± SD.…”
