Personalized dosimetry of 177Lu-DOTATATE: a comparison of organ- and voxel-level approaches using open-access images

Abstract: Lu-DOTATATE (Lutathera ® ) enables targeted radionuclide therapy of neuroendocrine tumors expressing somatostatin receptor type 2. Though patient-specific dosimetry estimates may be clinically important for predicting absorbed dose-effect relationships, there are multiple relevant dosimetry paradigms which are distinct in terms of clinical effort, numerical output and added-value. This work compares three different approaches for 177 Lu-DOTATATE dosimetry, including 1) an organ-level approach based on referenc… Show more

“…Although the fixed threshold method is simple, the appropriate threshold level strongly depends on the signalto-background ratio [52]. What is more, a single value might not be sufficient for every clinically relevant structure in the image [53]. This method is also strongly biased by the acquisition and reconstruction algorithms.…”

“…Image segmentation has found various applications in computer vision and has risen in popularity all over the globe. (Multi)organ segmentation is crucial in Monte Carlo (MC)‐based internal/external dosimetry using an anthropomorphic phantom to calculate average organ dose and so‐called organ‐specific S‐values in radionuclide imaging and therapy 5 . Moreover, in radiotherapy treatment planning, accurate delineation of target organs is commonly performed by an expert radiologist and/or radiation oncologist in order to maximize the dose delivered to the tumor and simultaneously minimize the radiation burden to the organs at risk (OARs) 6 .…”
