Portuguese / English: www.scielo.br/reeusp RESUMO As complicações agudas e crônicas enfrentadas pelo paciente diabético e sua família após a alta hospitalar podem ser consequência de deficiências no processo educativo ao longo da hospitalização e do preparo formal para alta. O objetivo deste estudo é apresentar uma proposta de planejamento da alta hospitalar do paciente diabético adulto. Foi realizada revisão de literatura sobre alta hospitalar da clientela em questão, selecionando-se artigos publicados de 2004 a fevereiro de 2009. Considerando a literatura, foi desenvolvido um impresso para nortear o planejamento da alta. Este abrange informações a serem colhidas e trabalhadas junto ao cliente, ao longo dos primeiros quatro dias de internação, considerando as necessidades individuais e o Autocontrole ineficaz da saúde. A alta precisa estar inserida no Processo de Enfermagem, uma vez que o enfermeiro tem papel fundamental na identificação das necessidades do paciente e família. O impresso auxilia a identificação das necessidades do cliente e das ações realizadas pela equipe. DESCRITORES RESUMENLas complicaciones agudas y crónicas enfrentadas por el paciente diabético y su familia luego del alta hospitalaria pueden derivar de deficiencias en el proceso educativo durante la hospitalización y de la preparación formal para el alta. Este estudio objetivó presentar una propuesta de planeamiento de alta hospitalaria del paciente diabético adulto. Se realizó revisión de literatura sobre alta hospitalaria de pacientes en cuestión, seleccionándose artí-culos publicados desde 2004 hasta febrero 2009. Según la literatura, se desarrolló un impreso para orientar el planeamiento del alta. Este incluye informaciones para recoger y trabajar junto al paciente, durante los primeros cuatro días de internación, considerando necesidades individuales y Autocontrol ineficaz de la salud. El alta necesita insertarse en el Proceso de Enfermería, toda vez que el enfermero tiene papel fundamental en identificación de necesidades del paciente y familia. Objetivou-se conhecer a percepção da mãe sobre a alta hospitalar e o cuidado do recém-nascido prematuro no domicílio após a primeira semana de alta. Pesquisa descritiva, de abordagem qualitativa, com uso da entrevista semiestruturada para coleta de dados, desenvolvida no ambulatório de seguimento do prematuro (follow-up) do Hospital da Rede Pública Estadual de Fortaleza, em outubro e novembro de 2011. As participantes foram nove mães de recém-nascido prematuro após primeira semana de alta. Aplicou-se análise do conteúdo. Os resultados apontaram para as categorias: Orientações sobre o cuidado com o prematuro durante a alta hospitalar; Sentimentos e dificuldades com a chegada do bebê no domicílio; e Apoio familiar no cuidado do prematuro. O estudo evidenciou que algumas mães não receberam orientações da equipe multiprofissional da neonatologia acerca do cuidado bebê após alta hospitalar, reforçando que elas levam o bebê para o domicílio sem estar preparadas para a nova realidade.

Conclusion-In the United States, hip fracture rates and subsequent mortality among persons 65 years and older are declining, and comorbidities among patients with hip fractures have increased. About 30% of people with a hip fracture will die in the following year, 1-3 and many more will experience significant functional loss. [4][5][6][7] The long-term consequences may be great as well. Some studies have shown excess long-term mortality even 10 years after an episode, [8][9][10][11][12] although other studies have only shown moderate increases in mortality. [13][14][15][16][17] Treating hip fractures is also very expensive.
