Perifoveal Exudative Vascular Anomalous Complex-Like Lesion as a Complication of Prepapillary Arterial Loops

Abstract: Perifoveal exudative vascular anomalous complex (PEVAC) and its expanded spectrum have been recently elucidated by multimodal imaging. Although there is some clarity about its characteristic features and natural history, the cause remains unknown. Herein, the authors describe a hitherto unknown association with prepapillary arterial loops and suggest a possible hypothesis for the development of a PEVAC-like lesion and its response to treatment with thermal laser. “…Treatment of PEVAC lesions with laser using CWMYL-577 had been reported previously by Ayachit et al 10 However, in this report the patient had a worse functional and structural outcome with persistency of intraretinal cysts, which could be explained by the different LASER parameters used. Mrejen et al, 11 also reported a patient treated with two sessions of thermal laser (spaced 7 months), with parameters similar to ours (40 ms, 100 mW and 100 µm spot size), and with good results, as well.…”

“…The pathogenesis of PEVAC is not clear, with some speculating ( 2 ) that it is a vascular anomaly of the macular retina and others suggesting that it is a reaction to the slow retinal blood flow between the ramus communicans and interarterial loops ( 8 ). Overall, greater attention should be paid to this disease to avoid misdiagnosis and to clarify its pathogenesis and treatment.…”

“…The pathogenesis of PVAC and its treatment are not yet established. [1][2][3]6,8 It was first identified in a healthy patient without any other vascular or inflammatory disease. However, in 2019, Venkatsh et al and Zhang et al identified PVAC in a diabetic patient who was unresponsive to anti-VEGF injections.…”
