Percutaneous Tracheostomy: Ciaglia Blue Rhino Versus the Basic Ciaglia Technique of Percutaneous Dilational Tracheostomy

Abstract: To assess practicability and safety of the Ciaglia Blue Rhino (Cook Critical Care, Bloomington, IL)-an extensively modified technique of percutaneous dilatational tracheostomy-50 critically ill adults on long-term ventilation underwent either new Ciaglia Blue Rhino or percutaneous dilatational tracheostomy in a prospective, randomized clinical trial.

“…In our study, there was no desaturation, accidental extubation, endotracheal tube or cuff puncture. The only significant complication encountered was fracture of tracheal ring during PercuTwist dilatation in 2 of our patients which has been reported with PercuTwist by Emmanuel Scherrer and with Ciaglia Blue Rhino technique by Christian Byhahn [8,12].…”

“…In 1990, Griggs et al further improved the technique by using the guidewire dilating forceps (GWDF), a modified Howard-Kelly forceps with blunt edges [7]. In [8]. This markedly reduced the complications.…”

“…Comparisons between the original Ciaglia and PercuTwist methods and between the Ciaglia, Griggs, and PercuTwist methods did not indicate a statistically significant difference in any outcomes. Comparison between the Blue Rhino and PercuTwist showed that only difficulty was statistically different (higher for PercuTwist), but Birbicer et al 24 did not report tracheal ring fractures, and Byhahn et al 22 did not report pneumothorax incidences.…”

“…In a comprehensive study of perioperative complications from the various dilatational percutaneous tracheostomy techniques, Byhahn et al [5] reported a 23% overall occurrence of complications, with a minimum of 6% for the Ciaglia Blue Rhino Ò (Cook Medical, Limerick, Ireland) dilator technique. Peri-operative complications most frequently associated with dilatational percutaneous tracheostomy techniques include hypotension, hypoxia, bleeding, resistance to the insertion of the cannula, peritracheal tube insertion (with false passage formation), damage to the posterior tracheal wall and fracture of tracheal rings (the latter accounting for about 25% of all complications) [5,6]. It is often believed that when performing dilatational percutaneous tracheostomies, tracheal rings are displaced, but remain intact.…”
