Which Percutaneous Tracheostomy Method Is Better? A Systematic Review

Abstract: BACKGROUND: The aim of this study was to assess the different methods of percutaneous tracheostomy in terms of successful performance of the tracheostomy as well as safety. Tracheostomy is the most common procedure performed on the airway for patients in ICUs. Lately, several methods of percutaneous tracheostomy (multiple dilator, progressive dilator, forceps dilation, screwlike dilation, balloon dilation, and translaryngeal) have been described, with theoretical advantages, but there is no consensus about whi… Show more

“…The traditional method of establishing a surgical airway involves an incision through the skin and the cricothyroid membrane into the tracheal lumen through which a tracheal tube is inserted. Many commercial kits are available intended for use in a CICV situation . Some of these kits are based upon the Seldinger technique where the cricothyroid membrane is punctured with a needle, a guidewire introduced into the tracheal lumen through the needle and a specially designed tracheal tube inserted over the guidewire.…”

“…For instance, Griggs’ technique provided addition with only a dilatation and under visual aid, compared with the Ciaglia multi-dilator technique. Fantoni technique offers intratracheal placement of the tube and Frova method decreases the risk of cartilage fracture [7]. Therefore, novel and current approaches to reduce complication rates and improve procedural success while using these techniques include visualization with fiberoptic bronchoscopy and determination of neck anatomy by ultrasonography.…”

“…In conclusion, comparisons between individual methods are not sufficient to clearly establish superiority of one over others due to incomplete case series or small sample sizes [7]. Therefore, comparison of Blue-Rhino method and other methods arises from the availability and widespread usability of the method.…”

“…Wenn auch die Einschritt-Dilatationstechnik die in Deutschland am häufigsten angewendete Technik ist und in 2 Metaanalysen leichte Vorteile im Vergleich zu den anderen Techniken gezeigt werden konnten [384,395], so lässt die derzeitige Datenlage keine allgemeine Empfehlung einer bestimmten Methode zu.…”
