Percutaneous Radiologic, Surgical Endoscopic, and Percutaneous Endoscopic Gastrostomy/Gastrojejunostomy: Comparative Study and Cost Analysis

Abstract: Technical success for gastrostomy is higher for FPG and SEG than PEG. Though PEG is the least costly procedure, the difference is modest compared with FPG. For gastrojejunostomy, FPG offers the highest technical success rate and lowest cost. Due to high costs associated with the operating room, SEG should be reserved for those patients undergoing a concurrent surgical procedure.

“…This corresponds to a superior technical success rate of 97% to 100% reported in literature for the IG technique [9][10][11][12]. Most conversions were because of anatomic difficulties such as scoliosis and high position of the stomach with a narrow subcostal angle, which are more likely to be present in neurologically impaired children.…”

“…Barkmeier et al [9] indicated in their report that PEG was the less costly procedure. However, this advantage was lost when the need for operating theater facilities and general anesthesia were factored in.…”

“…From a practical standpoint, several methods are available for tube feeding [7,11]: nasogastric tube, PEG, PFG and surgical endoscopic gastrostomy [2]. PEG feeding has a number of advantages, including greater mobility, comesis, and improved quality of life [14].…”

“…Nutrient intake is reduced by anorexia, nausea, eating and swallowing disorders, obstructive malignancies of the upper gastrointestinal tract, and radiationinduced mucositis. Percutaneous fluoroscopic gastrostomy (PFG) has been shown to be a safe and well-tolerated means of providing long-term enteral nutrition in such cases [2]. The literature contains numerous reports on use of PFG in head and neck cancer patients [8,16,17], but limited information is available on the impact of enteral nutrition [14] in head and neck cancer patients treated by concomitant radiochemotherapy (CRC).…”
