Tumor hypoxia and systemic toxicity seriously affect the efficacy of photodynamic therapy (PDT) and are considered as the “Achilles’ heel” of PDT. Herein, to combat such limitations, an intelligent orthogonal emissions LDNP@SiO2‐CaO2 and folic acid‐polyethylene glycol‐Ce6 nanodrug is rationally designed and fabricated not only for relieving the hypoxic tumor microenvironment (TME) to enhance PDT efficacy, but also for determining the optimal triggering time through second near‐infrared (NIR‐II) fluorescence imaging. The designed nanodrug continuously releases a large amount of O2, H2O2, and Ca2+ ions when exposed to the acidic TME. Meanwhile, under downshifting NIR‐II bioimaging guidance, chlorine e6 (Ce6) consumes oxygen to produce 1O2 upon excitation of upconversion photon. Moreover, cytotoxic reactive oxygen species (ROS) and calcium overload can induce mitochondria injury and thus enhance the oxidative stress in tumor cells. As a result, the NIR‐II bioimaging guided TME‐responsive oxygen self‐sufficient PDT nanosystem presents enhanced anti‐tumor efficacy without obvious systemic toxicity. Thus, the fabricated nanodrug offers great potential for designing an accurate cancer theranostic system.

Catalytic therapy can concurrently enhance therapeutic efficacy and decrease side effects by converting less toxic or nontoxic endogenous substances into various highly toxic reactive species (e.g., toxic radicals) to induce cell apoptosis/necrosis, which has been exploited to combat different types of diseases. As material science and nanotechnology evolve for application in disease therapies, a variety of catalytic biomaterials and nanomedicines have been elaborately designed and synthesized to mediate catalytic reactions and/or enhance the corresponding catalytic treatment efficacy. This review systematically summarizes, outlines, and discusses the state-of-the-art advances in the development and use of catalytic biomaterials and nanomedicines in diverse biomedical applications, particularly concentrating on their endogenous activators, exogenous stimuli (e.g., light, heat, ultrasound, and magnetic field), and the integration of endogenous and exogenous triggers to improve the efficacy of biomaterial/nanomedicine-enabled catalytic treatments. The biosafety and biocompatibility of various catalytic biomaterials and nanomedicines for biomedical applications are also discussed. Finally, the current challenges and future opportunities for advancement of catalytic biomaterials and nanomedicines are highlighted, aiming to boost their early fulfillment of practical clinical applications.

Oral administration is a widespread and convenient drug delivery approach. However, oral delivery can be affected by the complex digestive tract environment, including irregular tissue morphology, the presence of digestive enzymes, mucus and mucosal barriers, and spatiotemporal variance in physiological parameters. These obstacles can prevent the oral delivery of many therapeutics. To overcome these challenges, oral delivery devices can be designed with bioinspired compositions, structures or functions to make more drugs available for oral administration. Various bioinspired oral delivery devices have been developed by harnessing biological materials and living microorganisms, or by imitating biological structures and functions. In this Review, we discuss the design and modification of bioinspired oral delivery devices, examining engineering strategies to target specific tissues and applications. We highlight how key bottlenecks in oral delivery can be addressed through bioinspired designs, concluding with an outlook on the remaining challenges towards the clinical translation of bioinspired oral delivery devices.
