Background Orthodontically-induced root resorption is an iatrogenic effect and it cannot be examined regularly due to the harmful effects of sequential doses of radiation with more frequent radiography. This study aims to compare protein abundance (PA) of pre-treatment and during orthodontic treatment for root resorption and to determine potential early markers for root resorption. Methods Ten subjects (n = 10) who had upper and lower fixed appliances (MBT, 3 M Unitek, 0.022″ × 0.028″) were recruited for this study. Human gingival crevicular fluid (GCF) was obtained using periopaper strips at pre-treatment (T0), 1 month (T1), 3 months (T3), and 6 months (T6) of orthodontic treatment. Periapical radiographs of the upper permanent central incisors were taken at T0 and T6 to measure the amount of root resorption. Identification of changes in PA was performed using liquid chromatography-tandem mass spectrometry. Student’s t-test was then performed to determine the significance of the differences in protein abundance before and after orthodontic treatment. Results Our findings showed that all ten subjects had mild root resorption, with an average resorption length of 0.56 ± 0.30 mm. A total of 186 proteins were found to be commonly present at T0, T1, T3, and T6. There were significant changes in the abundance of 16 proteins (student’s t-test, p ≤ 0.05). The increased PA of S100A9, immunoglobulin J chain, heat shock protein 1A, immunoglobulin heavy variable 4–34 and vitronectin at T1 suggested a response to stress that involved inflammation during the early phase of orthodontic treatment. On the other hand, the increased PA of thymidine phosphorylase at T3 suggested growth promotion and, angiogenic and chemotactic activities. Conclusions The identified proteins can be potential early markers for root resorption based on the increase in their respective PA and predicted roles during the early phase of orthodontic treatment. Non-invasive detection of root resorption using protein markers as early as possible is extremely important as it can aid orthodontists in successful orthodontic treatment.

Several biologically active substances representing the bone deposition and resorption processes are released following damage to periodontal tissue during orthodontic movement. Biomarkers are by definition objective, quantifiable characteristics of biological processes. The analysis of saliva/salivary fluid and Gingival crevicular fluid (GCF) may be an accepted way to examine the ongoing biochemical processes associated with bone turnover during orthodontic tooth movement and fixed orthodontic treatment pain. Assessing the presence of these salivary physiological biomarkers would benefit the clinician in appropriate pain diagnosis and management objectively of various problems encountered during the orthodontic procedures and for better outcome of biomechanical therapy. Due to lack of standardized collection procedure, even though well accepted by patients, saliva is often neglected as a body fluid of diagnostic and prognostic value. A literature search was carried out in major databases such as PubMed, Medline, Cochrane library, Web of Science, Google Scholar, Scopus and EMBASE for relevant studies. Publication in English between 2000 to 2021 which estimated Saliva markers as indicators of orthodontic tooth movement was included. The list of biomarkers available to date was compiled and is presented in table format. Each biomarker is discussed separately based on the available and collected evidences. Several sensitive salivary and GCF biomarkers are available to detect the biomechanical changes occurring during orthodontic tooth movement and pain occurring during fixed orthodontic therapy. Further focussed research might help to analyze the sensitivity and reliability of these biomarkers or cytokines, which in turn can lead to the development of chairside tests to assess the pain experienced by patients during orthodontic therapy and finally the outcome of the fixed orthodontic therapy.

Orthodontically induced external apical root resorption considerations of root-filled teeth vs vital pulp teeth: a systematic review and meta-analysis

Introduction The purpose of this systematic review was to research the difference between root-filled teeth (RFT) and vital pulp teeth (VPT) in orthodontically induced external apical root resorption (EARR) and to offer suggestions for clinicians on therapeutic sequence and timing when considering combined treatment of endodontic and orthodontic. Materials and methods An electronic search of published studies was conducted before November 2022 in PubMed, Web of Science and other databases. Eligibility criteria were based on the Population, Intervention, Comparison, Outcome, and Study design (PICOS) framework. RevMan 5.3 software was used for statistical analysis. Single-factor meta-regression analysis was used to explore the sources of literature heterogeneity, and a random effects model was used for analysis. Results This meta-analysis comprised 8 studies with 10 sets of data. As there was significant heterogeneity among the studies, we employed a random effects model. The funnel plot of the random effects model exhibited a symmetrical distribution, indicating no publication bias among the included studies. The EARR rate of RFT was significantly lower than that of VPT. Conclusions In the context of concurrent endodontic and orthodontic treatment, priority should be given to endodontic therapy, as it serves as the foundation for subsequent orthodontic procedures. The optimal timing for orthodontic tooth movement post-root canal therapy is contingent upon factors such as the extent of periapical lesion resolution and the degree of dental trauma sustained. A comprehensive clinical assessment is essential in guiding the selection of the most suitable approach for achieving optimal treatment outcomes.
