Pelvic floor muscle assessment on three‐ and four‐dimensional transperineal ultrasound in women with ovarian endometriosis with or without retroperitoneal infiltration: a step towards complete functional assessment

Abstract: Women with ovarian endometriosis who were affected by DIE showed smaller hiatal dimensions than did women without DIE. Considering that PFM dysfunction in patients with DIE could cause pain symptoms and pelvic organ dysfunction, transperineal ultrasound could allow a more complete functional assessment and tailored therapy. “…It is acknowledged that LAM coactivation is often physiologically associated with Valsalva maneuver, especially in young women 23 . However, recent evidence has shown a correlation between LAM coactivation and some pathologies associated with chronic pelvic pain and pelvic floor hypertone, such as deep infiltrating endometriosis 24,25 . In the present study, we found a correlation between LAM coactivation and longer duration of the second stage of labor, but LAM coactivation was not associated with a higher risk of Cesarean delivery.…”

“…LAM contraction, rather than relaxation, during Valsalva maneuver (also called LAM coactivation) occurs in a substantial number of women and is believed to represent an occasional physiological accompaniment of a Valsalva maneuver. Recent research, however, has found an association between LAM coactivation and pathologically hypertonic pelvic floor. As far as we know, the effect of LAM coactivation on the outcome of labor has not been assessed previously.…”

“…Indeed, recent studies support the association between pelvic floor muscle hypertone and LAM coactivation. It has been shown that women with deep infiltrating endometriosis, who are known to have a considerably higher risk of pelvic floor hypertone, have almost a three-fold higher risk of LAM coactivation 39,40 . We also confirmed an association between LAM coactivation after visual feedback and longer second stage of labor.…”

“…The association between pelvic floor musculature and pelvic pain is recognized particularly for certain conditions, such as hypertonic pelvic floor, of which muscle spasm is a key part. Recently, ultrasound data in women with endometriosis have also demonstrated alterations in pelvic floor morphometry, particularly in the subgroup of deep infiltrating endometriosis. Generalized pelvic pain may also cause a reflex increase in pelvic floor muscle spasm, the extent of which is unknown.…”
