The aim of this study was to evaluate the demographic and clinicopathologic data of salivary gland tumors managed at a tertiary referral medical center in Turkey. The data of 510 patients with salivary gland tumors managed during the period of January 1984 to May 2012, were reviewed. Only primary neoplasms derived from salivary glands were included. Out of 510 neoplasms, 352 (69.0 %) were classified as benign and 158 (31.0 %) were classified as malignant. There was a male predominance and male:female ratio was 1.23 (281/229). The most common location was parotid gland (372/510, 72.9 %) followed by minor salivary glands (97/510, 19.0 %) and submandibular gland (40/510, 7.9 %). The malignancy rates were 21.5, 40.0, and 56.7 % in parotid, submandibular, and minor salivary glands locations, respectively. The most common location for minor salivary gland neoplasms was oral cavity (61/97, 62.9 %). Pleomorphic adenoma (PA) was the most common histopathological type (45.3 %) in the whole study group and also among pediatric patients. The most common malignant neoplasms were adenoid cystic carcinoma (39/510, 7.6 %) and mucoepidermoid carcinoma (5.7 %). Salivary gland tumors are more common in men. The malignancy rate is almost three times higher in neoplasms derived from minor glands when compared to parotid gland. PA is the most common histopathological tumor type in all locations and in all age groups.

ÖZPediatrik yaş grubunda tükrük bezi tümörleri nadir görülmektedir. Bu kitlelerin histopatolojisi çeşitlilik göstermektedir. Lezyonların çok azını maligniteler oluşturur. Yetişkinlerde olduğu gibi, pediatrik yaş grubu malign tükrük bezi tümörleri en sık parotis bezinde görülür. Yine yetişkinlerde olduğu gibi en sık görülen histopatolojik tip mukoepidermoid karsinomdur. Adenoid kistik karsinoma ise daha ender rastlanır. Sekiz yaşında kız çocuğu, sağ parotiste uzun süreden beri mevcut olan şişlik şikayeti kliniğimize getirildi. Öyküsünde şişliğin dışında ek bir şikayetinin olmadığı öğrenildi. Hastanın yapılan muayenesinde sağ preauriküler bölgede yaklaşık 2x1cm boyutlarında, mobil, ağrısız, sert kitle tespit edildi. Bunun dışında muayenesinde herhangi bir patoloji saptanmadı. Yapılan ultrasonografi sinde(USG) sağ parotis yüzeyel lojda ovoid, yaklaşık 1,5x1,5cm boyutunda nodüler lezyon gözlendi. Hastanın yapılan manyetik rezonans görüntülemesinde(MRG) sağ parotis yüzeyel lobda yerleşimli, kontrast tutulumu gösteren kitlesel lezyon gözlendi. Hastaya ince iğne aspirasyon biyopsisi(İİAB) yapıldı. Biopsi sonucu granülomatöz hastalık olabileceği yönündeydi. Buna yönelik yapılan tetkiklerinde herhangi bir granülomatöz hastalık bulgusuna rastlanmadı. Hastaya eksizyonel biopsi planlandı. Genel anestezi altında süperfi cial parotidektomi uygulanan hastanın histopatolojik tanısının adenoid kistik karsinom olarak raporlanması üzerine, hastaya total parotidektomi uygulandı. Postoperatif dönemde 28 gün boyunca 60Gy/gün radyoterapi verildi. Hasta takibinin 2 yılında ve nükse ait herhangi bir bulgu şu ana kadar gözlenmedi. Geç dönem rekürrens ve metastazlara yatkın olan bu hastalığın tedavisinin esasını cerrahi oluşturmaktadır. Perinöral invazyon yapması hastalığın rekürrens oranlarını arttırmaktadır. Postoperatif dönemde radyoterapi uygulanması ile rekürrenslerin önüne büyük ölçüde geçilebilir. Pediatrik yaş grubunda nadir görülmesine rağmen, hızla tanı konulup tedavisinin planlanması gereken hastalığın parotis kitleleri ayırıcı tanısında çocuklarda da mutlaka akılda tutulması gereklidir.Anahtar Sözcükler: Adenoid kistik karsinom, Parotis bezi, Pediatrik hasta ABSTRACT Salivary gland tumors rarely occur in the pediatric population. These tumors can present with various pathological diagnoses. Malignancies constitute only a small part of these lesions. Pediatric malignant salivary gland tumors are most commonly seen in the parotid gland as in adults. Mucoepidermoid carcinoma is also the most common type of parotid tumor in childhood as in adults. Adenoid cystic carcinoma is seen more rarely. An 8-year-old girl presented at our clinic with a long-standing painless swelling of the right parotid. On physical examination, a mobile solid mass about 2x1 cm in size was noted on the right preauricular area. Magnetic Resonance Imaging (MRI) showed a mass in the right parotid gland.
