Pediatric bipolar disorder: evidence for prodromal states and early markers

Abstract: Background-Childhood bipolar disorder remains a controversial but increasingly diagnosed disorder that is associated with significant impairment, chronic course and treatment resistance. Therefore, the search for prodromes or early markers of risk for later childhood bipolar disorder may be of great importance for prevention and/or early identification.

“…1 There is increasing agreement on clinical phenotypes of children and adolescents who are at elevated risk for progression to fully syndromal BD. [2][3] Among children of parents with BD, subthreshold "high-risk" forms of the disorder can be detected as many as 10 years prior to onset of BD I or II. 3 Significant controversies exist about the diagnostic boundaries of early-onset BD, but agreement is substantial that BD and its subthreshold antecedents have a considerable impact on general functioning and quality of life.…”

“…Despite the relatively large number of studies describing prodromes in psychiatric disorders, for example, in bipolar disorder [20] or depression [21], there are limited data in the available literature about the percentage of children and adolescents with somatic symptoms equivalent to the symptoms indicative of a mental illness. Some light on the somatic and psychological condition of schoolchildren was thrown by a survey of health behavior conducted among 200 000 adolescents in 43 countries in Europe and North America [22].…”

“…Another article (Geller et al, 2009) also mentions zero cases of PTSD in a diagnostic list for forty-seven 14 year old PBD subjects in a neuroimaging study. A further article (Luby & Navsaria, 2010) had PTSD in a citation title but PTSD/trauma was not mentioned in the text.…”
