Pectoralis Major Musculocutaneous Flap With Nipple-Areola Complex in Head and Neck Reconstruction

Abstract: Pectoralis major musculocutaneous flap remains the workhorse tool for head and neck reconstruction. Flap failure in head and neck reconstruction is a devastating complication with a high morbidity and mortality. Inclusion of nipple-areola complex on the skin paddle stabilizes the blood circulation in the skin island of the pectoralis major musculocutaneous flap. A modified use of pectoralis major musculocutaneous flap with nipple-areola complex on the skin island was performed in 11 male patients in head and n… Show more

“…Not placing the skin paddle too low on the rectus muscle fascia or including the nipple areolar complex inside the PMMC flap are design modifications that improve cutaneous portion survival. 27 Practical methods also exist for improving flap survival when the flap is pale or congested. Arterial insufficiency can also be improved when the suture line is not excessively tight, and the intravenous volume expansion can improve arterial insufficiency.…”

“…Standard technique for harvesting the PMMC flap was implemented. In certain cases with large external defect requiring larger skin paddle, the nipple areola complex (5 patient) was harvested with the skin paddle to stabilise the vascularity of the flap [3]. The age of the patients ranged from 40 to 65 years.…”
