Patterns of Proinflammatory Cytokines and Inhibitors during Typhoid Fever

Abstract: Cytokines and inhibitors in plasma were measured in 44 patients with typhoid fever. Ex vivo production of the cytokines was analyzed in a whole blood culture system with and without lipopolysaccharide (LPS). Acute phase circulating concentrations of cytokines (±SD) were as follows: interleukin (IL)-IP, <140 pg/ml.; tumor necrosis factor-a (TNFa), 130 ± 50 pg/mL; IL-6, 96 ± 131 pg/ml.; and IL-8, 278 ± 293 pg/ml., Circulating inhibitors were elevated in the acute phase: IL-l receptor antagonist (IL-IRA) was 2304… Show more

“…The pathology observed in hu-SRC-SCID mice infected with S. Typhi resembles human typhoid in a number of respects, including hepatic Kupffer cell swelling and splenic granulomatous inflammation with epithelioid macrophages and multinucleated giant cells (Mallory 1898;Ayhan et al 1973;Bharadwaj et al 2009). Elevated serum levels of the cytokines IL-6, IFN-g, and TNF-a are also observed, as in patients with typhoid fever (Butler et al 1993;Keuter et al 1994). Engraftment of human hematopoietic stem cells into a different mouse background, Rag2 IL2rg null mice, has also been shown to render mice susceptible to S. Typhi infection.…”

“…Of these, the TNF gene, which encodes the pro-inflammatory cytokine TNF-α, is a strong candidate. Keuter et al (1994) measured TNF-α levels in typhoid fever patients and found that the production of this cytokine was lower in the acute phase of the disease than in convalescence. Bhutta et al (1997) have reported an association between circulating TNF-α levels and typhoid fever severity and more recently House et al (2002) showed that low ex vivo production of TNF-α was associated with a delayed recovery.…”
