Background Botulinum toxin type A injection is a common aesthetic treatment that can be used alone or in combination to rejuvenate the entire face and neck. Objectives To assess the safety and efficacy of a full‐face approach with botulinum toxin type A. Methods This was a single‐center, retrospective review of data from 189 patients receiving full‐face and neck treatment with onabotulinumtoxinA in the following areas: glabellar lines, crow's feet lines, forehead lines, bunny lines, sagging nasal tip, gummy smile, marionette lines, mentalis, masseter, and lateral/anterior platysma. Results The mean age was 56.6 ± 9.2 years, and 94.7% were women (n = 179). Patients received a total of 394 full‐face treatments (mean: 2.1 per person; range 1‐7). The mean quantity of onabotulinumtoxinA injected was 113.8 ± 17.6 units per treatment (range: 50‐180 units). The most frequently injected areas were the glabella/crow's feet/forehead (n = 383; 97.2%), platysma (n = 378; 95.9%), bunny lines (n = 283; 71.8%), and chin (n = 120; 30.5%). Six patients (3.2%) experienced complications: five cases of unwanted diffusion from the mentalis into the depressor labii inferioris and one case of right eyelid ptosis after injection in the upper third. All were minor and none appeared to be related to overall dose. Conclusions Full‐face and neck treatment with onabotulinumtoxinA can be performed without substantial safety concerns, often leading to excellent results. This approach may become increasingly prevalent in the coming years.

BotulinumtoxinA (BoNT-A) is now widely established for the main approved indication of reducing glabellar lines, and is also widely used off-label to improve the appearance of wrinkles and lines in other parts of the face. The number of aesthetic procedures continues to increase as the patient population becomes more diverse, in particular with increasing numbers of people of color and men. Further developments in treatment may continue to expand the audience for BoNT-A by making procedures more comfortable and by delivering a more natural, less static appearance. These may be achieved through use of combinations of BoNT-A with other aesthetic procedures, tailoring the dose of toxin to the patient’s muscle mass or by using novel injection and application techniques. Beyond amelioration of facial lines, encouraging results have been seen with the use of BoNT-A to improve the appearance of hypertrophic and keloid scars and even to prevent them. Studies have been conducted with scars in various parts of the body and further research is ongoing. Dermatological and other medical uses for BoNT-A are also active areas of research. Injections of BoNT-A have been shown to reduce signs and symptoms of acne, rosacea, and psoriasis, to reduce neuromuscular pain, and to bring about significant improvements in a number of rare diseases that are caused or exacerbated by hyperhidrosis. This paper reviews these new uses for BoNT-A, looking at the rationale for their use and discussing the results of published case studies and clinical trials. These areas have shown great promise to date, but more and larger clinical studies will be required before these treatments become a clinical reality. To this end details are also provided of clinical trials currently listed in the main clinical trials database to highlight research areas of particular interest.

Background:Indians constitute one of the largest population groups in the world. Facial anthropometry, morphology, and age-related changes in Indians differ from those of other ethnic groups, necessitating a good understanding of their facial structure and the required aesthetic treatment strategies. However, published recommendations specific to Indians are few, particularly regarding combination treatment.Methods:The Indian Facial Aesthetics Expert Group (19 dermatologists, plastic surgeons, and aesthetic physicians with a mean 15.5 years’ aesthetic treatment experience) met to develop consensus recommendations for the cosmetic facial use of botulinum toxin and hyaluronic acid fillers, alone and in combination, in Indians. Treatment strategies and dosage recommendations (agreed by ≥ 75% of the group) were based on results of a premeeting survey, peer-reviewed literature, and the experts’ clinical experience.Results:The need for combination treatment increases with age. Tear trough deficiency is the most common midface indication in Indian women aged 20–40 years. In older women, malar volume loss and jowls are the most common aesthetic concerns. Excess medial soft tissue on a relatively smaller midface precedes age-related sagging. Hence, in older Indians, fillers should be used peripherally to achieve lift and conservatively in the medial zones to avoid adding bulk medially. The shorter, wider lower face requires 3-dimensional correction, including chin augmentation, to achieve increased facial height and the oval shape desired by most Indian women.Conclusions:These recommendations give physicians treating Indians worldwide a better understanding of their unique facial characteristics and provide treatment strategies to achieve optimal aesthetic outcomes.
